A 4yo male neuter Australian Cattle Dog is referred to you on a Friday afternoon...
- He was seen with a brown snake 24 hours earlier
- He has had a 24 hour history of progressive ataxia and
- Progressive increase in respiratory effort
BUT!
- PT/ APTT is NEGATIVE
- Snake Venom Detection Kit (SVDK) is NEGATIVE
- AND he is showing signs of respiratory fatigue...

Did you know that it's not the absolute value of the initial plasma lactate in dogs with GDV that is a prognostic indicator?

That's right! It's actually the REDUCTION in the plasma lactate following fluid resuscitation...

Certainly, studies have shown that survival is more likely when initial plasma lactate is 6mmol/L BUT if the plasma lactate reduces by > 40% following initial fluid resuscitation then it indicates a better chance of survival in these patients.

If plasma lactate does not reduce >40% despite fluid resuscitation, aggressive investigation should be pursued and in patients where hyperlactemia persisted > 24-48hrs, many did not survive.

What looks like hypovolemic shock but is not?

What delays perfusion parameters but doesn't need volume resuscitation? In fact, fluids are the one thing that these patients DON'T need...

Cardiogenic Shock!

Cardiogenic shock results from inadequate cellular metabolism caused by cardiac dysfunction... The most common cause of systolic dysfunction is Dilated Cardiomyopathy but can also be severe MMVD or hypertrophic obstructive myopathy. The most common cause of diastolic dysfunction can be due to pericardial effusion & cardiac tamponade, HCM or tachyarrhythmias.

This little patient presented in severe cardiogenic shock and respiratory distress due to severe enlargement of his left and right atria due to HeartWorm disease!

He responded brilliantly to supplementary oxygen therapy, frusemide and pimobendan... he was discharged home 24 hours later to represent to his primary care vet for immiticide treatment.

Got Polytrauma? Take a breath... ReadyVet has got you covered!!

This poor puppy sustained severe wounds after being attacked by a big dog and presented collapsed and in severe hypovolemic shock and with wounds penetrating into the abdomen...

Don't panic... Polytrauma is as easy as ABC!
And it all comes down to oxygen delivery: DO2 = (Hb x SaO2 x 1.34)
A = make sure that you have a patent airway + provide supplemental oxygen
B = breathing
C = circulation! Treat shock to improve the oxygen carrying capacity of Hb!

When fluid resuscitation doesn't work...

So you have given crystalloid boluses to your shock patient but they haven't worked... your patient hasn't improved and your perfusion parameters are still abnormal... so what next??

Remember that a full blood volume for dogs is 90ml/kg and cats are 60ml/kg... if your boluses are close to reaching half a blood volume (ie. 45ml/kg in dogs and 30ml/kg in cats) and your patient is still not responding...

- Consider cardiogenic shock: does your patient have jugular pulses or a heart murmur?
- Consider distributive shock ie. Anaphylaxis or Sepsis...
- Consider obstructive shock ie. GDV or pericardial effusion...
- Consider ongoing blood losses ie. Internal or external haemorrhage...

ReadyVet not only calculates half a blood volume for your patient, it ALSO remembers how much fluid you have given them and will warn you when you get close to half a blood volume - so you can focus on your patients and not on the maths!

SHOCK – Part 4.

So you know that your patient is in shock… you’ve started fluid resuscitation… but how do you know when to STOP?

Repeated boluses of crystalloid/ colloid/ blood products administered intravenously will normalise Resuscitation end points by re-establishing depleted intravascular volume. After each bolus, the patient is reassessed for normalisation of end point resuscitation variables:

1. Normalisation of perfusion parameters:
- Mucous membrane colour: Pink
- Capillary refill time (CRT): 1-2 seconds
- Heart rate: Dogs 90-140bpm; Cats 180bpm
- Re**al temperature: > 37.5 oC
- Mentation: alert
- Pulse quality: SAP 100mmHg; MAP 70-80mmHg

If the patient’s perfusion parameters normalise, you can continue with maintenance/ replacement rates.
BUT: when they do NOT normalise… its time to STOP and reassess your patient…

2. When its not just hypovolemic shock…

If you have delivered >40-50ml/kg crystalloid boluses ie. half a blood volume for dogs and 20-30ml/kg in a cat and your resuscitation end points have not yet normalised… it is time to consider other causes of shock refractory to fluid resuscitation including:
• Septic shock.
• Cardiogenic shock.
• Uncontrolled haemorrhage.

The ReadyVet app not only keeps track of the total volume of your fluid boluses, it will warn you when you approach these and ask you to consider what else could be wrong with your patient to avoid the potentially harmful consequences of fluid overload as well as delaying further, potentially life-saving treatment.

SHOCK – Part 3

The indications WHEN to perform fluid resuscitation in shock patients is clear… but when are fluids NOT indicated in patients with shock?

The ReadyVet Veterinary Calculator app has got you covered – not only does it remind you to check that your patient does not have the existing comorbidities where fluid resuscitation is contraindicated, it provides helpful examples of what to use instead.

Does your patient have evidence of:
• Heart disease?
• Renal failure?
• A haemabdomen?
• Traumatic brain injury?
• A severe burn injury?
• Are they a large breed dog?

Patients with these co-morbidities require either hypotensive resuscitation/ limited volume or NO fluid resuscitation… and ReadyVet will calculate the volume of these treatments for a given patient weight as well as the total volume to give.

SHOCK – PART 2

Resuscitation End Points are early indicators of occult shock and can be used to guide resuscitation efforts.

Resuscitation End Points reflect adequate blood flow to vital organs. In our first post on SHOCK we discussed what deficits in Perfusion Parameters looked like for the different forms of shock.

‘Normalisation’ of these Perfusion Parameters indicates when you can stop shock fluid therapy and commence maintenance/ replacement fluid therapy.

Can you remember what ‘normal’ Resuscitation End Points are?
Resuscitation End Points:
1. Mucous Membrane colour: Pink
2. Capillary Refill Time (CRT): 1-2 seconds
3. Heart Rate:
- Dogs: 90 – 140 bpm (depending on size)
- Cats: 180 bpm
4. Re**al temperature: >37.5OC
5. Mentation: Alert
6. Pulse quality:
- Systolic Arterial Pressure (mmHg): 100mmHg
- Mean Arterial Pressure (mmHg): 70-80mmHg

How do we use Resuscitation End Points to guide our Fluid Resuscitation Therapy?
• Repeated boluses of crystalloid/ colloid/ blood products administered intravenously will normalise Resuscitation End Points by re-establishing depleted intravascular volume.
• After each bolus the patient is reassessed for End Point Resuscitation Variables.

SHOCK!

Can you remember the 4 different types of shock?
1. Hypovolemic:
Eg. Loss of water & electrolytes (Renal/ Gastrointestinal), Loss of blood
(Haemorrhage)

2. Distributive (Vasodilatory):
Eg. Anaphylaxis, Severe inflammation/ SIRS/ Sepsis, Neoplasia.

3. Cardiogenic (Reduced Cardiac Output):
Eg. Left sided heart failure: evidenced by tachypnoea, pulmonary oedema,
cyanosis
Right sided heart failure: evidenced by Jugular pulses, pleural effusion, ascites.

4. Obstructive (Impairment to blood flow):
Eg. GDV, pericardial effusion, pneumothorax.

Deficits in Perfusion parameter are suggestive of SHOCK.

Clinical signs of SHOCK include:
1. Mucous Membrane colour:
- Pale: Hypovolemic/ Cardiogenic/ Obstructive shock
- Injected pink/ red: Distributive shock

2. Capillary Refill Time (CRT):
- Delayed capillary refill time: Hypovolemic/ Cardiogenic/ Obstructive shock
- Rapid capillary refill time: Distributive shock.

3. Heart Rate: Tachycardia

4. Peripheral temperature: Cool/ Hypothermic

5. Mentation: Decreased

6. Pulse quality:
- Decreased: Hypovolemic/ Cardiogenic/ Obstructive shock
- Bounding pulses: Distributive shock.
