SHOCK – PART 2
Resuscitation End Points are early indicators of occult shock and can be used to guide resuscitation efforts.
Resuscitation End Points reflect adequate blood flow to vital organs. In our first post on SHOCK we discussed what deficits in Perfusion Parameters looked like for the different forms of shock.
‘Normalisation’ of these Perfusion Parameters indicates when you can stop shock fluid therapy and commence maintenance/ replacement fluid therapy.
Can you remember what ‘normal’ Resuscitation End Points are?
Resuscitation End Points:
1. Mucous Membrane colour: Pink
2. Capillary Refill Time (CRT): 1-2 seconds
3. Heart Rate:
- Dogs: 90 – 140 bpm (depending on size)
- Cats: 180 bpm
4. Rectal temperature: >37.5OC
5. Mentation: Alert
6. Pulse quality:
- Systolic Arterial Pressure (mmHg): 100mmHg
- Mean Arterial Pressure (mmHg): 70-80mmHg
How do we use Resuscitation End Points to guide our Fluid Resuscitation Therapy?
• Repeated boluses of crystalloid/ colloid/ blood products administered intravenously will normalise Resuscitation End Points by re-establishing depleted intravascular volume.
• After each bolus the patient is reassessed for End Point Resuscitation Variables.
• If the patient responds to End Point Resuscitation variables, then you can continue with maintenance/ replacement rates.
