CASE STUDY - MAISIE (Equine Metabolic Syndrome)
A little while back we received a most beautiful THANK-YOU card from Maisies best friend "Abby". Here is Maisie's story.
Maisie is a 14 year old pony mare who has had recurring laminitic episodes despite her owners best efforts in ensuring that appropriate management needs were met. During her most recent laminitic episode in March this year, Maisie became increasingly sore, withdrawn and dull. She was not bouncing back from this flare up like she previously had. Her owners were deeply concerned about Maisie's quality of life and so called SHEC for one last effort to get Maisie feeling herself again. Dr Imogen Thomas was the treating Veterinarian for this case.
Radiographs were taken of all 4 feet for the purpose of assisting the farrier with remedial trimming and shoeing as part of the care going forward. Maisie had previously been tested for cushings 2 years ago where she returned a negative test. This time it was decided that her insulin should be tested as well as a retest for cushings (Equine ACTH levels). The Equine ACTH test returned another negative, however her blood insulin levels maxed out at >300 mU/L. This was an indication that Maisie had insulin dysregulation, a key component of Equine Metabolic Syndrome (EMS). As a result Maisie's dietary management was further tweaked and treatment with Ertugliflozin (a sodium-glucose co-transporter 2 inhibitor) was commenced.
Six months on we are happy to report that Maisie is comfortable, her spark has returned, and she is even being ridden again! She is bright, cheeky and challenging her little rider keep up with her quick trot again, which is an excellent report.
