Question❓: Which organ indicators should be evaluated before anesthesia?

Be careful to screen for common geriatric diseases, such as heart, kidney, liver, central nervous system and tumor diseases. In general, older animals are sedated at lower doses than usual because they may have delayed clearance. Careful monitoring is required during anesthesia, and related complications should be addressed as soon as possible, otherwise organ function may be affected. These complications include hypotension, hypoxemia, hypercapnia, and hypothermia.

🔍🔍Very juvenile animals are those less than 5 months of age, whose livers are not fully functional. It takes longer for them to metabolize many anesthetics; in addition, their glycogen utilization is insufficient, and hypoglycemia is prone to occur during anesthesia, so blood sugar levels should be monitored and added if necessary. When the blood glucose is lower than 60g/dl, 2.5% (25mg/ml) glucose can be added to the balance solution for intravenous infusion, and infused at the maintenance speed during normal anesthesia.

Be aware of the breed of animal, some breeds require special considerations for anesthesia management.
*Brachycephalic breeds: There may be upper airway obstruction, resulting in hypoxemia and cardiopulmonary arrest.
*Boxers, Sighthounds: Sensitive to acepromazine, may develop severe hypotension and cardiac arrhythmias.
*Collies: Poor metabolism of certain sedatives and analgesics, may prolong awakening.
*Small dogs: Hypoglycemia, hypothermia, and hypotension may occur; intraoperative monitoring equipment does not match.
*Large dogs: Small body surface area, some drug doses should be appropriately reduced.

Dental Disease in Rabbits

Dental disease is seen often in rabbits Including overgrown cheek teeth, spurs (sharp points), malocclusion and elongated roots.

Unfortunately, this can cause pain and discomfort when eating causing appetite issues and can often lead to gut stasis (a slowing or stopping of movement of material through the digestive tract).
Diagnostic testing such as imaging (x-rays or CT scan) and endoscopy (HD angled camera for detailed visual inspection) is required for a full dental exam so we can assess the crowns and also the roots.

We recently saw a rabbit with malocclusion resulting in these overgrown incisors. The incisors were removed and she is now eating well with just her cheek teeth.

This second rabbit had a sharp spur on her upper premolar that we saw on her physical exam. Using the endoscope we could see the ulceration that was caused by this. This tooth (and others that were overgrown) were filed down with an electric burr under general anaesthesia. She recovered quickly and she was happy to go home the same day!

Both of these rabbits were eating normally on presentation which shows it is not always obvious if our furry friends are uncomfortable. We encourage rabbit owners to bring their pets for frequent health checks by an exotic animal vet so dental disease and other conditions can be picked up and treated early.

What is the difference between an indwelling needle and an ordinary needle?

1. The materials are different. The retaining needle is mainly composed of a catheter and a steel needle. The catheter will wrap the steel needle, and the ordinary needle is the steel needle.

2. Different uses. After the retaining needle is inserted into the blood vessel, the steel needle should be withdrawn, and only the flexible tube should be retained. Before infusion, it should be rinsed with normal saline, and the tube should be sealed with isotonic saline or diluted heparin solution after infusion. The commonly used needle is directly pierced into the blood vessel, and the needle is pulled out directly after the injection.

3. The time of application is different. The retention time of the indwelling needle is relatively long, usually about 3 to 4 days. Under normal circumstances, the general needle retention time is short, mostly 2 to 4 hours.

4. Different activity restrictions. The retaining needle adopts a flexible tube, which has less damage to the blood vessels and generally does not affect the activities of the hand. Two-month-old cat "Coco" suffered an accidental fracture of the distal femur.
He was rescued by kindhearted people and brought to the pet hospital. After conditioning, he underwent surgery today. Because it was a juvenile animal and the fracture site was located at the distal femoral growth plate, the rushpin dynamic cross-pin technique was used to fix it so as not to affect the bone growth. Coco recovered in a stable state after surgery.

How to expand pets' veterinary health care!

With the increased interest in pet ownership, researchers predict U.S. pet healthcare spending will increase 33 percent between 2019 and 2029.

Unfortunately, the number of new veterinarians entering the field each year usually increases by just 2.7% annually, while roughly 2,000 vets retire from the profession each year.

An estimated shortage of nearly seven thousand veterinarians in the U.S. is causing issues for pet-owners and veterinarian clinics across the country.

The 32 Veterinary medical schools produce about 3,200 graduates each year-- not nearly enough to cover the number of doctors who retire and the increase in demand from pandemic pets.

Measures we could implement together to increase the access to veterinary care in the US:

Expand veterinary school class sizes from typically 60 to 160.

Open additional veterinary schools with innovated technology and improved curriculum.

Mimic human health success with midlevel practitioners Like licensed animal health technicians.

Expand the scope of practice of credentialed veterinary technicians and veterinary assistants.

Educate veterinarians about the efficient use of telemedicine.

Reduce labor pressures and provide valuable patient care by increasing technology like new software and wearables technology for pets.

Expand numbers of states that recognize reciprocity in allowing licensed veterinarians to move to a new state easily using the help of AVMA and American association of veterinary state boards (AAVSB) to have a national veterinary license.

Simplify the process for international veterinary graduates to become licensed in the USA by modifying the ECFVG program and introduce more digital form.
