Did you ever wonder why there is a big bulge over one of the canine teeth? This condition is called alveolar bone expansion and is a form of osteomyelitis and periodontitis that affects cats. Sometimes you can even pluck out the tooth with your fingers. But the third image here shows you what is hidden beneath. This bone is diseased and must be removed after making a flap.

How do you close the extraction site after extraction of 109/209? Making a pedicle flap is one option, but you get dangerously close to the salivary ducts when releasing the flap. Also, you risk that the flap partly covers the cusp of 110/210.
For the past decade, this is how I have been closing this site, as described in the Oral and Maxillofacial Surgery book by Verstraete, Boaz, and Lommer.
You should focus on getting the gingiva snug around the distal root of 108/208 to recreate the gingival collar.
The rest can be left to granulate in - happens within two weeks or less.
The risk of dry socket is extremely rare.

X-rays will underestimate bone loss because 40% of the bone needs to be gone before it's visible.
Deciding whether to extract a tooth or not also relies heavily on the clinical examination.

If the pulp is exposed, it is just a matter of time before problems arise.
Dogs and cats show no obvious clinical signs when the toothache kicks in.
Do not tell the owner that “we’ll keep an eye on it” or “we will wait until it becomes a problem”
If you say that you are letting the pet down.

Sometimes you get surprised by what you find! I have seen this orientation of the second molar before but never had to extract one until now.
It was scary, but I like a good challenge 😎

Any embedded/impacted tooth should be extracted to avoid a dentigerous cyst forming.
I do not advise waiting and seeing if the cyst forms or not, because the older the dog gets, the more difficult it will be to get out. In a seven-month-old dog like this, the tooth was pretty mobile.

Human maxillofacial surgeons are very concerned about nerve pain after extraction if the inferior nerve is in contact with a root. Sometimes the root even hooks around the nerve.

This root was not in contact with the nerve ( I could feel the bony cavity after extracting the tooth).

Gingival Enlargement! Overgrown gingiva is a problem because it creates pseudopockets that collect debris and dirt and will lead to periodontitis. I recommend to do a biopsy to rule out malignancy. And remember that the term “gingival hyperplasia” is a histological diagnosis, so until we know the exact diagnosis, just write “Gingival enlargement” on your chart 😊

Malocclusions are not as common in cats as in dogs, but they certainly do occur.
Be sure to look at the caudal mandible when doing an awake oral exam in cats, as the sharp prominent tips of the upper premolars may contact and traumatize the mucosa of the mandible adjacent to the lower premolar and molars. If it is not addressed in time, it can lead to l lesions in the oral mucosa.
Treatment includes reshaping the crowns (odontoplasty) of the upper 4th premolars (and sometimes the 3rd premolar too) to shorten and blunt them. A bonded sealant is applied to seal the exposed tubules. If that doesn’t resolve the lesion, then they will need to be extracted.
Make sure not to remove too much dentine so the pulp is exposed. There needs to be at least 0,5 mm dentine covering the pulp. Use a lateral X-ray to guide you 🙂

310/410 are prone to developing periodontal disease, and we often extract them to stop the progression of bone loss on the distal root of 309/409.
Often, it halts the progress, but sometimes it will progress.
-
I theorize that it may be due to the lack of a proper gingival collar protecting the distal root of 309/409 when extracting 310/410 without adressing the gingiva. This was a large breed dog with tough gingiva and it was not possible to close the gingiva snugly around the distal root of 409 by just approximating the buccal and lingual gingival.
-
This is a technique how you can try and recreate the full collar of attached gingiva around the distal root by making a triangular flap with a vertical releasing incision mesial to 411.
-
A key to success is of course also properly cleaning the root surface from calculus before closing. I recommend using a mirror during cleaning to see the area properly.

Have you ever seen a tongue like this and wondered what it was? Rubbing the sublingual surface and frenulum against the mandibular incisors when the dog pant can cause the mucosal epithelium to become hyperplastic and hyperkeratinized. Treatment was as simple as extracting the lower incisors. The difference is stunning! ☀️

The periodontal ligament of the buccal roots of 109/209 can be difficult to evaluate with the standard view. I recently tried this method developed and published by , and it worked surprisingly Well. This image I took was only my second attempt. Definitely something I will implement for future COHATS 😊
