This is the same pug 5 hours after surgery....and....silence....cannot even hear her breathe. Very happy for this little one, but manybshort faced breeds suffer all their lives from this breed specific condition, and the public needs to be aware and put commercial pressure on breeders to eliminate this cruel problem.

Severe barchycephalic obstructive airway syndrome (BOAS) in this pug - elongated soft palate + everted laryngeal saccules + stenotic nares. Surgery involved soft palate shortening, laryngeal saccules resection and stenotic nares correction. Its was amazing to see the dramatic change immedietly post-op! This surgery needs to be done as early as possible to prevent progression and to enable a much better quality of life. Very satisfying to see the change we have made in her life....

BOAS in a pug....markedly stenotic nares - the cartilage wings excision from inside the nostrils is key to an open airway, just opening the exterior nares is not sufficient.

So yesterday was Large Dog day, today was Small Dog day....two small breeds, each with a very steep TPA of 38 and 43 degrees. TPLO can still be done with these, as long as adequate thickness of the tibial tuberosity is ensured.

Two large Rottweilers, one yesterday and one today. Important to apply the largest plate possible to minimize chances of fragments moving, as these heavy large dogs are at a greater risk of implant/bone instability or failure.

Lateral patella luxation, in the Cockapoo breed especially, is NOT the mirror image of medial patella luxation. A simple tibial tuberosity transposition which is very effective in most medial luxations often fails in these cases. Its not entirely clear, but some papers suggest quadriceps net direction of pull to be one of the major causes, which subjectively to me seems supportive of my personal experience in dealing with many of these cases over the years. Applying a RidgeStop prosthetic trochlear groove ridge laterally, with a lateral arthrotomy extending a bit more proximally than usually required, and leaving it as a "lateral release" has worked very well in my subjective experience (hopefully a paper on retrospective cases will be published in the future, working on it...). The hypothesized mechanism by which this seems to work is that by a more proximal incision of the lateral capsule the direction of the net pull of the quadriceps is altered, effecting patella re-alignment, and the laterally placed RidgeStop acts as a very effective stabilizer that guides the patellar correctly tracking along the groove. The advantage of a RidgeStop implant is that it extends proximally and is much more efficient than even a block sulcoplasty (and less traumatic).

Finally published in the highly regarded Journal of Feline Medicine and Surgery. Long term followup of the patented Ridgestop (Orthomed UK) implant in cats. Ridgestop is definitely my go-to technique for the vast majority of feline patela luxations and has been for almost a decade. Worth a read as can make your life so much easier in these cases.

Objectives The aim of this study was to assess the complication rates and long-term outcomes, as well as to refine case selection criteria for cats undergoing t...

Distal humeral comminuted fracture repair...transcondylar screw used in the distal hole of the medial plate to ensure no intercondylar fraccture develops, plus medial plate (all locking screws except the trancondylar one). Even though we already do these complex fractures, always good to see new approaches and implants...

On the odd occasion that we still do Lateral Sutures (small breeds only), its important to drill the hole in the (elusive) so-called isometric point, which means at the closest point to the cranial cruciate ligament's insertion point on the proximal tibia. In truth there is no perfect isometric point, but can come close. How do we know our hole was at the ideal spot? if the tension on the lateral suture remains tight and is maintained uniformly throughout full range of motion, and flexion/extension is smooth.
