At S.A.S. we strive to perform advanced cutting edge precise surgical procedures like this 10 months old Dachshund today which had growth deformity called Pes Varus in its left hind tarsus, caused by unequal growth of the distal tibial growth plates, causing the foot to turn inwards and thus limping on that limb.
A 3D image was created based on a CT scan, which enabled accurate planning of the optimal osteotomy line (with the professional help of Bill at Vet3D Ltd). Then based on this, a pre-op and post-op bine model was 3D printed, as well as custom jig used intra-operatively to guide the saw angle for accurate results. The distal segment is quite small and Dachshund bones are quite contoured as well, and therefore two metal custom bone plates were also 3D printed, as otherwise it would be veru difficult to contour a plate to the bone shape, and would also risk entering the joint space. The 3D planning and custom plates also ensure that the screws from the two plates don't hit each other, which is especially tricky in the distal small fragment.
At the end, synthetic bone graft granules (N2 UK ltd) were used to fill the opening wedge gap, and this can be seen quite well on the craniocaudal xray view.
Great case, am sure Dachi is gonna do well!

Pre and post-op xrays of the fracture in the previous video, where an NXP pin was used to distract and align the fragments, and left in as regular i.m. pin. (Plate used was a mono-axial 2.4mm locking plate from N2).

NXP pin makes light work of a complex comminuted femoral fracture repair.....Easy one handed and accurate fragment distraction bringing the two main fragments to the perfect pre-measured length, and then keeping it accurately in position, enabling leasurely and stress-free plate placement. (best seen if video zoomed-in at fracture site). Once the plate is applied, the NXP pin not only functions as a standard i.m. pin increasing resisistance to bending forces, but also adds some resistance to compression forces, and there is no risk of pin "back-out" which can often be an annoying complication. NXP pins really are a game-changer in long-bone fracture repair, turning challenging repairs into easy and stress free ones, with increased accuracy and much reduced surgery times, a simple but very powerful new tool in the surgeon's hand, all at negligible extra cost.

Cat with a complex high energy trauma.Y-Fracture, one of the most challenging fracture types to fix. Here there were splinters from the main fragments, making accurate reconstruction even more challenging. An olecranon osteotomy was done to enable better visualization of the fragments and this enabled accurate reconstruction which is essential for joint function. Two mono-axial 2.0mm combined locking plates (N2) were used bilaterally, and a 2.0mm trans-condylar locking screw in positional fashion to secure the condyles after compression with a bone holder forceps (locking screws have a thicker shaft diameter than cortical screws, hence are stronger and therefore preferable).

Sacroiliac subluxations....even though the margin of error in positioning and directing the drill hole in the sacrum is quite small, freehand "eye-balling" it can work very well, by aiming the drill towards the surgeon's finger positioned on the contralateral side.
