Condylar fractures are a common problem in the equine athlete. Fractures develop because of repetitive injury and weakness of the bone. Meticulous lameness investigation in conjunction with good radiographic technique are essential to identify these injuries before they become a complete fracture. Standing repair of these fractures under sedation and perineural nerve block allows horses to return to competition.

After treating horses at Musselburgh racecourse Eugenio drove down to Aintree so he could teach on the Association of Veterinary Racecourse Surgeons casualty management course. Treating vets on British racecourses have to attend this course every five years to be eligible to work at a racecourse. Pictured is Eugenio explaining how to deal with the recumbent racehorse. 🏇🏇🦷🦷Fracture of the incisor teeth of the maxilla or the mandible are not unusual in young horses due to their tendency to play with hard objects such as hay managers. Sometimes the horses spook and their teeth get caught and become fractured. These fractures can be successfully repaired using advanced dental techniques, standing sedation and regional nerves blocks. 🏇🏇🦷🦷

A unilateral nasal discharge can be a result of primary sinusitis that occurs in horses when the drainage angle is blocked. The sinuses become filled with pus that shows as a fluid line on xrays. The pus can be cleaned away from the sinuses by drilling a small hole and placing a tube into the sinuses to help unblock the sinus drainage angle. Many cases can be resolved with this surgical treatment alone but more complicated secondary sinusitis involving the teeth or sinus cysts often need computed tomography (CT) to confirm diagnosis and assist management planning.

Keratomas are benign space-occupying tumors that alter the normal architecture of the hoof capsule. The lameness can be mild to moderate, depending on the degree of impingement on the sensitive laminae. This case is an unusual spherical keratoma on the lateral aspect of the sole. Early detection of clinical signs and standing MRI can allow early recognition of condylar fissure fractures that are radiographically silent. These can be managed to allow a successful return to racing even if they are bilateral.

Proximal phalanx (P1) fractures usually present as severe non-weight bearing lameness during or after exercise. In this case clinical and radiographic examination revealed an intra-articular comminuted fracture of P1. Under general anaesthesia, using arthroscopic guidance the fracture was reduced and stabilized with 3 lag screws. These cases have a good prognosis for return to racing, allowing horse to achieve previous level. Rapid diagnosis and treatment are essential to guarantee a success.

LASER surgery can be used for the removal of skin tumours such as sarcoids, melanomas or mast cell tumours. This has the effect of heating cells around the incision, thereby killing sarcoid cells that may be remaining in the skin. The tissue remaining at the base of the removed sarcoid can also be treated with the laser, again with the aim of killing remaining sarcoid cells.

LASERs can be used trans-endoscopically to facilitate minimally invasive surgical treatment of many equine upper respiratory tract conditions (‘wind’ problems). It can also be used to treat lesions affecting other inaccessible parts of the body such as uterine cysts and urethral strictures

Solar penetrations are an emergency in equine practice, many vital soft tissue structures can be affected.
This is an interesting case where a traumatic solar puncture resulted in a pedal bone sequestrum (piece of dead bone) and a collateral cartilage infection (Quittor)
Surgical intervention and removal of the infected cartilage and pedal bone sequestrum resulted in a good outcome.
Another great team collaboration between vets and farriers.

Ocular trauma can be tremendously challenging in veterinary medicine.
Fracture of the orbit, damage to the globe, uveitis and eyelid lacerations are common consequences of trauma.
Precise apposition of the eyelids is essential to recover proper eye function.

Carolyne worked at the Dick Vet Equine Hospital for 20 years and brings a wealth of horse handling and nursing experience. We are tremendously lucky to have someone like Carolyne to joint the team!!.When she’s not helping horses Carolyne is devoted to her two Beagles, Rufus and Dave.

Standing surgery has revolutionised the way we operate on horses. I have been tremendously lucky to have as a mentor one of the leading experts in the field of airway surgery, Prof. Padriac Dixon. A colleague and a friend from which I have had the privilege to learn from. This is another example of a procedure that we used to do under general anaesthesia and now we do standing using sedation.
