12 infectious in-patients (isolation ward) with 24 hours in patient monitoring and critical care. We can manage the following diseases and conditions:

A bronchoscopy study in dogs and cats involves the use of a bronchoscope to visually examine the airways (trachea and bronchi) and, if needed, collect diagnostic samples. This procedure is often performed to investigate respiratory issues, such as chronic coughing, wheezing, labored breathing, or unexplained respiratory infections.

At Animal Doctors we can do this procedure:

Steps in a Bronchoscopy Study:
1. Pre-procedure Preparation:
• Sedation or Anesthesia: Since bronchoscopy is invasive, the cat is placed under general anesthesia to minimize stress and discomfort.
• Pre-anesthetic Tests: Blood work and imaging studies (such as X-rays) may be performed to evaluate the patient overall health and identify abnormalities.
2. Bronchoscopy Procedure:
• Insertion: A thin, flexible or rigid bronchoscope is inserted through the patient mouth and down the trachea.
• Examination: Out veterinarian visually inspects the airways for signs of inflammation, tumors, foreign bodies, or other abnormalities.
• Sample Collection:
• Bronchoalveolar Lavage (BAL): A saline solution is introduced into the lower airways and then suctioned out. The fluid is analyzed for cells, infectious agents, or other abnormalities.
• Biopsies: Tissue samples may be taken if masses or suspicious areas are identified.
3. Post-procedure Care:
• The patient is monitored closely during recovery from anesthesia.
• Antibiotics, anti-inflammatories, or other medications may be prescribed depending on findings.

Indications for Bronchoscopy:
• Chronic or recurrent respiratory symptoms (e.g., coughing, wheezing)
• Suspected airway obstructions (e.g., tumors, foreign objects)
• Persistent infections unresponsive to treatment
• Diagnostic follow-up for conditions like asthma, bronchitis, or pneumonia

Risks and Considerations:
• Complications: These can include minor bleeding, airway irritation, or, rarely, anesthesia-related risks.
• Specialized Equipment: Bronchoscopy requires expertise and specialized equipment, often available at referral centers and practices.

At Animal Doctors we value safety and we make sure our diagnostic imaging is of medical value. We take radiographs of pets under general anesthesia when needed, safety is crucial for both the pet and the veterinary staff on our practice.

Here are some key features why we do this:

1. Patient Positioning and Monitoring

• Minimize Movement: Secure the pet with positioning aids (sandbags, foam pads, etc.) to reduce movement, preventing repeated exposure.
• Maintain Anesthetic Monitoring: Ensure the pet’s vital signs are closely monitored, as anesthesia affects heart rate, respiration, and other critical functions.
• Consider Minimal Sedation: If general anesthesia is not essential, minimal sedation or alternative restraint methods may reduce anesthetic risk.

2. Radiation Safety for Staff

• Distance and Shielding: Stand at a safe distance or behind protective barriers. Use lead aprons, gloves, and thyroid shields to protect yourself.
• Limit Exposure: Use the lowest exposure time and intensity necessary to obtain a clear image.

Canine bladder stones, or uroliths, are mineral accumulations in a dog’s bladder that can cause discomfort, urinary issues, or even blockages. The exact causes of bladder stones vary, but they often involve a combination of genetic predisposition, diet, and certain metabolic conditions.

Causes of Canine Bladder Stones

1. Diet: Certain minerals and nutrients in a dog’s diet can contribute to stone formation. High amounts of calcium, magnesium, ammonium, or phosphates can increase the risk of certain types of stones.
2. Urinary Tract Infections (UTIs): Infections can alter urine pH, promoting stone formation.
3. Genetics: Some breeds, such as Dalmatians, Bulldogs, and Schnauzers, are predisposed to certain types of bladder stones.
4. Metabolic Disorders: Issues like hypercalcemia (high blood calcium) or liver disease can also make a dog more prone to developing stones.
5. Low Water Intake: Dehydration or low water consumption can lead to more concentrated urine, which is a risk factor for stone development.

Types of Bladder Stones

• Struvite Stones: Often associated with UTIs.
• Calcium Oxalate Stones: More common in certain breeds, often due to dietary and genetic factors.
• Urate Stones: Associated with liver issues and common in Dalmatians.

Surgical Treatment

When bladder stones are causing symptoms or posing a risk of blockage, surgery is often recommended. The procedure, known as a cystotomy, involves the following steps:
Preparation: The dog is placed under general anesthesia. Incision and Stone Removal: The surgeon makes an incision in the abdomen and bladder to remove the stones. Bladder Flush and Closure: The bladder is flushed to remove any remaining small fragments, then closed with sutures.

Post-Surgical Care and Expectations

1. Hospital Recovery: Dogs often stay at the veterinary hospital for 24–48 hours for monitoring and pain management.
2. Medication: Antibiotics are given to prevent infection, and pain relievers are provided to keep the dog comfortable.
3. Dietary Changes: The vet may recommend a specialized diet to prevent future stone formation based on the type of stone removed.
4. Water Intake: Encouraging increased water intake is essential to reduce urine concentration and minimize the risk of recurrence.
5. Rest and Activity Restrictions: Limiting activity for at least a couple of weeks allows the incision to heal. Regular leash walks are fine, but more vigorous activity should be avoided.
6. Follow-Up Testing: Regular urine tests and occasional imaging (like X-rays or ultrasounds) may be recommended to check for any new stone formation.

Long-Term Management and Prognosis

With proper management, many dogs recover well from bladder stone surgery. The risk of recurrence can often be minimized through dietary changes, increased water intake, and regular veterinary check-ups. However, some types of stones, like calcium oxalate, have a higher recurrence rate, so diligent monitoring and preventive care are essential.

By following the vet’s recommendations on diet, hydration, and medication, most dogs can return to a normal, healthy life after recovery.

Max, a one-year-old Pomeranian, was presented to us displaying lameness and was diagnosed with bilateral grade 3-4 medial patella luxation.

We proceeded with surgery on the left patella first due to its more severe clinical presentation.

X-rays confirm the pre- and post-surgery repair, showing the patella successfully repositioned in the groove through trochleoplasty. Additionally, we performed tibial tuberosity transposition and utilized pins and a tension band for optimal stabilization.

Recurrent urinary tract infections or UTI can be due to MRSA bacteria or antibiotics resistance.

UTI episodes includes increased in urinary frequency, hematuria or blood in urine, difficulty to urinate and fishy odour to the urine.

The patient was medically treated with oral antibiotics at first and second episode and got better but a reoccurence of UTI was noted. Blood clots with bladder wall hyperplasia were appreciated on ultrasound scan.
Possible cancer aside from urinary bladder hyperplasia was discussed with the client.

Surgical management via cystotomy and bladder wall scraping was indicated for this case. Collection of biopsy for histopathology to rule out cancer and soft tissue culture and sensitivity to identify the bacteria and treat it with specific antibiotic as needed.

On surgery a ventral abdominal approach was performed, extending from the umbilicus, parapreputial to the p***s. The bladder was identified, isolated, and exteriorized. A longitudinal incision was made in the ventral bladder wall, allowing for the evacuation of urine.
Upon inspection, the bladder mucosa appeared grossly abnormal and hyperplastic with bleeding into the mucosal surface. A hatspoon was used to scrape the bladder mucosa, yielding a large amount of bloody mucinous material.
Samples were collected during this procedure for tissue culture and sensitivity and urinary bladder histopathology. The bladder was then thoroughly lavaged with warm saline.
Bladder closure was accomplished in two layers.
The abdominal cavity was lavaged with warm saline and was closed routinely.

The patient is now hospitalised for urine outflow and antibiotic therapy and pain management.

Alfie, a senior male Alaskan Malamute, is currently receiving treatment for a blood parasite infection. He has been responding positively to the treatment since it began 3-4 weeks ago.

We are happy to report that he is showing signs of improvement and his energy levels have returned to normal.
