It's with a really heavy heart that I announce that my Nash Rambler took his last breath today. We knew his battle was going to be a hard one to fight and boy did he fight! When his original diagnosis extended creating megaesophagus we knew that aspiration pneumonia was going to be a major complications. Despite the last month on antibiotics pneumonia won the battle. Last week was a really good week and this weekend he took a hard turn for the worse.

Nash was only 2! He was a rambunctious high energy dog that was so full of love. 2 years on earth with him was not long enough. It truly is a mystery how this disease happened and took hold turning our world upside down.

I appreciate everyones understanding of last minute cancellations and rescheduling when Nash needed immediate care and for all your kind words. Please take the time to let your fur babies know every day how much you love them because you never know when that last breath will be taken.

Fly high Nashy Rambler and run like crazy! Until we meet again❤️

Hi, everyone!

I just wanted to update you all on whats been going on with Nash and I

Nash was a typical high energy Aussie who you could not burn out. This spring his energy levels started to plummet and we associated it to the fact that he was maturing and turning 2 June 30th. That was not the case!

He absolutely loved being outside to the point you had to force him to come in. In July it was the opposite. We would have to drag him out. Towards the end of July he began drinking an insane amount of water and peeing it out for a good 5-10 minutes. Seriously he peed that long! And he became very lethargic some days he couldn't keep his eyes open. Off to the vets he went where he was diagnosed with Diabetes Insipidus which means he was either not making the hormone to concentrate his urine or his kidneys were unable to use the hormone. If left untreated he would have ended up severely dehydrated and death was possible. He started on hormone treatment and immediately his urine was concentrated again.

In comes symptom #2. His right eye was staying dilated while the left did the usual constrict with light and dilate with low light. His eyes wouldn't return to normal which prompted a referral to a neurologist which was about 5 days to get in. They performed an MRI and spinal tap. The MRI showed that his pituitary gland was largely diffused, thickened and rounded. The doral region, pituitary stalk/hypothalamus was also involved. Basically inflammation from an autoimmune condition that was destroying the parts of the brain that controls the endocrine system and the optic nerves to the right eye.

The diagnosis of Hypophysitis an extremely rare disease. So rare that in 2017 there were only 5 documented cases. Nash makes case #6! Of the other 5 one died suddenly of an Addison crisis, 1 did not have all that Nash has and was not followed beyond 6 months. 3 were euthanized and the diagnosis was found with autopsy. So when we say rare it is extremely rare that treatment and prognosis is really unknown.

With that diagnosis came secondary diabetes insipidus, secondary hypothyroidism, secondary Addisons and secondary right parasympathetic III dysfunction (right eye).

He is on a high dose of Prednisone, thyroid meds, hormone to concentrate his urine. In comes another complication!

Nash starts this horrific cough. It was loud! Off to our amazing primary vet who did X-rays etc and did not see pneumonia or anything. We both checked in with the neurologist and got no where. He was on some cough suppressant in thoughts that it was just tracheal irritation. We had a follow up appointment on Monday the 19th with the neurologist so we knew the neurologist would be able to pinpoint better what might be going on. Yeah that didn't happen!

Now mind you he is on a high dose of Prednisone. With that came the muscle wasting of the left side of his face. Basically it's a skull with hair. Hard to see in pictures but to pet him on that side is hard! So his cough changed to more of an aspirating kind of hack leaving puddles of saliva, water and the occasional bits of food all over the place. Monday's follow up was just labs and they didn't think this new symptom had anything to do with his diagnosis. We couldn't even talk to the neurologist nor would she see him.

We walked out the door and called our primary vet who also agreed that was not ok. She started him on antibiotics for caution against aspirating pneumonia. It was clear now that he has a swallowing disorder. Wether it's from the muscle damage from the Prednisone or nerve and muscular damage from this super rare autoimmune disease.

Our awesome and amazing vet Lori Hassinger Baldwin took Nash's case home with her to study it at night and on her day off Wednesday went above and beyond contacting teams of neurologist who agreed this is a high priority issue and the old neurologist seriously dropped the ball. We saw a different neurologist yesterday (9/22) and boy was she so much better! She diagnosed him with megaesophagus which is another rare condition, put him on sildenafil (Vi**ra) to help relax the sphincter between the esophagus and stomach to help food pass through easier. And with this we have to feed Nash in a bailey chair (a bailey chair sits the dog straight up and down to allow gravity to move the food down as his muscles are too stretched to do it on their own) and he has to sit in the chair for 20-30 minutes after eating (as you can imagine he is not a fan😅) she also is starting him on a low dose of chemo to help suppress his immune system even more and then slowly start working down to a much lower dose on the prednisone. His new neurologist says that this could also be cancer but they are unable to biopsy his pituitary gland as it is invasive and the the only other way that could possibly tell is if they did a second MRI to see if It has grown (meaning most likely cancer) or shrunk meaning we are on the right track I passed on the MRI this time around (they are crazy expensive) and am focusing on the more concerning problem right now which is the megaesophagus

Now with the swallowing issue (megaesophagus) there is still the risk of him aspirating on his saliva which can cause aspirating pneumonia so we will have to be on the look out for that as well

So life with a special needs dog has really changed! We have several reoccurring alarms set on our phones for med times. Routines have changed! With all this being said in the future I may have to reschedule appointments on short notice and I am going to try and shorten my days as feeding him and giving him his medication is time consuming I may also cut back on taking new clients

Sorry for any inconveniences this may cause but I appreciate your understanding

This has been a tough summer for us especially him

Thank you🙂
