BEHAVIOR DETERMINATION: Level 1
Recommendations: No young children (under 5)
Recommendations comments: No young children: Due to the behavior seen in the care center, we feel that Puffy may be intimidated by young children. She needs a slow approach and time to warm up. She should never be forced to interact with new people, and should be allowed to greet and initiate an interaction at her own pace. Older, gentle children should have an in-depth interaction prior to adoption.

DVM Intake Exam

History: Found tied to a fence, brought in by police
Subjective: Alert, walked well on leash
Observed Behavior - Tense, sat still for exam, very quiet, didn't move much even for ear plucking
Evidence of Cruelty seen - None
Evidence of Trauma seen - None
Objective
BAR-H, MMs pink and moist, BCS 7/9
EENT: OS microphthalmia or anophthalmia, large amount of dried discharge at lateral canthus. OD nuclear sclerosis, no discharge. AU moderate stenosis, lots of fur in ear canals, black waxy discharge. No nasal discharge noted.
Oral Exam: Stage 3/4 periodontal disease, gingival mass growing over all maxillary incisors. Fractured 104, 204, 304 near root. 404 isn't fractured but the distal surface of the tooth is abraded away.
PLN: No enlargements noted
H/L: Heart murmur grade 4/6 PMI left base. NSR, CRT < 2, Lungs clear, eupnic, no coughing or sneezing.
ABD: Non painful, no masses palpated
U/G: Female, mature v***a, no discharge, mature ni***es, no masses.
MSI: Distal legs are urine-soaked and smell like urea. Dermal mass in antecubital fossa - approx 2 cm dia, pink, irregular surface, large amount of saliva staining in fur around mass. Smaller mass < 1 cm dia on left hip. Ambulatory x 4, mild muscle wasting on hind legs. Skin free of parasites.
CNS: Mentation appropriate - no signs of neurologic abnormalities
Re**al: Normal externally
Assessment:
1. Heart murmur
2. Chronic otitis, ear canal stenosis and fur
3. Dental disease, fractured teeth, suspect pt chews/pulls on bars with her teeth to cause the abrasion
4. Masses on left forearm and left hip, gingival mass - suspect pt licks/chews at the LFL mass
5. Microphthalmia/anophthalmia
6. Geriatric
7. Urine-stained
Plan:
1. Plucked fur from ears, cleaned both ears. Clean ears q3 days, monitor
2. Cleaned discharge from face and masses
3. Recommend chest rads +/- echocardiogram with permanent veterinarian
SURGERY: Permanent waiver due to age

There are many reasons for this like a hold or an eval has not been conducted yet or the dog is rescue-only... the list goes on...
