The CT findings present an inflammatory process including the prostate and the urethra (prostatitis/urethritis). These would match with the reported clinical symptoms. Signs of an abscess formation and/or a perifocal peritonitis are currently not recognized. Secondary bacterial infection is common. Therapy should achieve prostatic involution (castration, hormone therapy). Non-radiopaque calculi cannot be fully excluded. The CT study reveals a dentonasal fistula of the triadan 104 secondary to severe chronic periodontitis. Chronic regional osteitis is noted as well as severe regionally destructive rhinitis within the right nasal cavity. Extraction of the triadan 104 is recommended. Airway sampling could be considered as well to allow for antimicrobial treatment of the rhinitis based on the sensitivity spectrum if indicated. Note the moderate periodontitis with partial loss of the lamina dura of the triadan 204 which is likely to require extraction as well.

The appreciated nasal mass is consistent with primary nasal neoplasia, presenting a highly aggressive biological behavior with perforation of the cranial fossa. Differentials include adenocarcinoma, squamous cell carcinoma, transitional cell carcinoma, lymphosarcoma, other. Ultrasound guided FNA sampling of the extension of the mass into the left orbital cavity or rhinoscopy including biopsy can be used as advanced diagnostic tests. Based on the results of the advanced diagnostic tests, the chances of radiation therapy can be discussed with oncologist. The Adam tumor stage is T4.
