I provide a Custom Made Service Dog or Therapy Dog ID Card

The ID Card Size is 3 3/4" by 2 1/4" (about the size of a standard business card) heavily laminated making the card stiff and durable. I prefer this over the plastic "credit card" printed cards that shows wear with time and it's a great convenient size to carry in your wallet without taking up much space. The information needed for customizing is:

Dog's name
Handler/Owner's name
City, State Abbreviation and Zip (i.e. San Diego, CA 92128)
digital picture of your dog
choose Service or Therapy dog
choose color of the "Service" or "Therapy" text red, blue, green or any color you want (red is the default)

The back of the card has U.S. Once I receive your information, your card will be in the mail within 24 to 48 hours. If you have a rush, please advise so I can provide the fastest turn around and optional expedited shipping. Please Note: I am not involved with the certification process of Service Dogs. I am providing a service to make it convenient for Service Dog Owners to carry identification which may help them in public situations.
