Jyothy's dear pet Ammu came to us with dragging left hind leg, and in serve pain. She was missing for couple days time, and returned like this. After stabilizing, pain management we imaged her ( X-ray DR). Ammu's Thigh bone was fractured ( Distal femoral physeal fracture- SH type 1). After stabilizing sugery was performed. ( Approached the distal femur from an incision on the craniolateral aspect of the stiffle joint. Reduced the fracture and stabilized using crossed pins)

Ammu was placed on protective bandages, and medicines. She was inpatient for a few days, and returned home.

Ammu is gradually walking back to her normal life. Postoperative care in veterinary orthopedics is quite challenging, but not an excuse though. Post Operative period is 4-6 weeks. Lets celebrate life 😊.

Inhalant Anaesthesia in Birds

This is a rescued parrot. The parrot had been badly attacked by a cat causing severe damage to the neck region. Stabilized the bird, and Surgically corrected the wounds.

Gas anesthesia is as indispensable part in procedures, and even for physical examination and collection of samples( Blood ) in birds which are not tamed.Isoflurane and Sevoflurane are most commonly used.

Isoflurane. Induction is 3-4%, and maintainance at 1-2 %. O2 flow rate of 200 ml/ kg/ min ( min 0.5- 1 L). Induction in 30 sec to 2 min. Recovery in 3-5 Minutes.

We use heated table tops during the entire procedure set to 40 0C ( or use of hot water bag would be great.

Cover them in rolled towels during unduction, also during recovery to prevent damages.

Subungual squamous cell carcinoma( SCC) in dogs

SCC is a malignant tumor in the epidermal layer of skin. Subungual SCC seen as a swollen single digit, nodule, raised white skin, ulceration and so on. Shadow, 5y old Rottweiler came to us with swollen digit which hadn't been healed by use antibiotics for long period.

Dx: Diagnosed the condition by, excisional biopsy of the afected digit. Punch biopsy can also be done at early stages. Large-breed dark-coated dogs ( Labrador Retrievers and Rottweilers) are more prone to SCC digits.

Rx/ Treatment : In Subungual SCC removal of the affected digit is the best way out. Metcheck ( checking for metastasis ) was done, and no metastasis cloud be found.

Prognosis / Outcome : Dogs with Subungual SCC have a survival rate of 95% more than a year, and 74 % more than 2 years. Most of the pets will lead a nornal life for long period, if there is no metastasis at the time of diagnosis.

Pemphigus Foliaceus in Cat

Kannan, Persian, Male Cat and his parent Kiran were equally worried when they came to us. Kannan was hairless with crusts, and pustules all over his body. Pemphigus Foliaceus is the most common autoimmune skin disease in cats.

Dx / Diagnosis : Punch biopsy of skin using punch biopsy needle. Confirmation by presense superficial acantholysis, and excluding relevant differential diagnosis.

Rx / Treatment : Immunosuppression, and symptamatic treatment to concurrent coditions are the key elements. Steroids and other novel adjuant therapies are beneficial. Kannan was successfully managed on prednisolone and abx

Outcome / Prognosis : Good in cats depending up on comorbidities and response to treatment. Remission in 4-8 weeks. However, relapses may occur in 60 -70 % of the cases.

Feline Pseudomycetoma ( Dermatophytic mycetoma

Eva 1 year old female persian cat was presented with a fluctuating mass on her back. Bimal her parent was worried about this mass. After the frontline treatment, we decided to excise the mass, and do biopsy.

Cause : Feline dermatophytic mycetomas are uncommon deep dermal and subcutaneous fungal diseases caused by dermatophytes.

Dx: Diagnosed by culture / excisional biopsy/ cytology . This case was proved by biopsy after visualising the hyphae in the crushed granules. ( Missed picture of yellow granules in gel like substance )

Rx : Surgical removal ( Two solitary massses) followed by treatment with Itraconazole in this case.

Outcome / Prognosis : Solitary masses like in Eva can be cured by excision and treatment. Relapses and future infections are possible in ulcerated conditions.

Cherry eye in Dogs ( Prolapse of Nitictans gland )

Seen as a red bulging at the base of the eye. Beagles, Cockerspaniels, Bull dogs are over represented.

Diagnosis : from symptoms and appearance

Causes : Breed predisposition, laxity of eye structures

Treatment : Surgical replacement of the pr*****ed nictitans using Morgan's Pocketing method. Removal can develop dry eye( KCS), and loss of vision as remarkable portions of the tear is produced from this gland

Prognosis / Outcome : Earlier surgeries give the best results. 20-50 % of recurrance in chronic / inflamed cases. Our experience suggests naso lacrimal duct flush prior to surgery gives good outcome. 2 cases pr*****ed even after third surgery.
