Yesterday I posted about when to do scrotal ablation procedures, and this is one of those cases, as severe trauma to the area can necessitate removal of the sac along with the te**is 😬🩺

Here’s how I do mine:
✅ Start with an elliptical incision, wide enough to fully encompass the sc***um
✅ Bluntly dissect through the subcutaneous tissue, isolating the spermatic cords
✅ Double ligate those cords using absorbable sutures like PDS or Vicryl, check and double-check before transecting the cords.
✅ Close subcutaneous layers with continuous sutures and finish the skin with a tension-free closure

*GRAPHIC WARNING* 🚨 Have you guys seen a case like this before?

This is a rare auto-immune disease in German Shepherds called Peria**l Fistulas 😱

Often in these guys you’ll see an intact middle age male German Shepherd come in for chronic, purulent, smelly, ulcerating, wounds in the a**l region and surrounding skin that start off looking like boils and often get infected (ie. pics) 😰

How do you treat this condition? Medical management is what I like to do with immune-modulating drugs and improvement can often be seen quickly; though it can take still take several months of treatment to get this persistent disease under control 😬

The drugs I like to treat these with are:
✅ Topical Tacrolimus
✅ Topical Animax
✅ Oral Cyclosporine
✅ Oral Prednisone
✅ Oral Cephalexin
✅ Probiotics ( 🙂)

Unfortunately some cases do return and require intermittent treatment for life. Keeping the fur in this area short along with consistent bathing and cleaning can help as well 🧼
