Luna has now been officially cleared for Chemo.

Prior to the go ahead, despite being already diagnosed with SCL, ideally all cats need to have a heart scan. Why? Both chemo and prednisolone affect the heart, and weaken it…so if there are signs of congestive heart failure, sadly…chemo is off the table.

Despite being diagnosed with HCM 7 years ago due to being on high dose prednisolone for IBD, the results are not too bad.
Ejection fraction and contractility is absolutely normal and within healthy ranges. Aorta is normal, as is actually everything else…only left ventricular wall has increased to 8mm (7mm last year and has been 6.1 for a few years prior). She does have a mitral leak, but no fluid around her heart. So I think, it’s safe to say…it wasn’t too bad of a result.
And to top it off…not only is she good with blood tests (sadly, through experience), she didn’t even need assistance for the heart scan. She just stayed on her sides and let the vet do her thing.

Meds have been ordered, and she will have her first blood test in 2 weeks time after starting her meds to ensure the blood panel is looking ok. If her WCC and RBC are too low, dose will be adjusted. And then it’s 6 monthly scans of her intestines and liver, and heart.

But…Sadly, her diarrhoea has come back…and the only food that was helping is no longer helping…this is the reality of her condition. And until her meds kick in…well, there’s a lot of paw baths.

Here we go…Her operation went well…and she’s been recovering since. She’s still not allowed to get up to any monkey busi...

Her operation went well…and she’s been recovering since. She’s still not allowed to get up to any monkey business or try and beat her personal best km/h on the cat wheel.

Her recovery tent is still up, but at the moment she uses it when she wants some time out, away from everyone.
Don’t worry…she’s not been left alone. First week, all outings have been supervised, and night time…I was on the floor next to her tent. Now she’s sleeping with me, without moving off the bed at night.

We recieved the results yesterday, we are still to discuss next steps with the internal medicine specialist…but unfortunately, it is Small Cell Lymphoma.
Good news, was that it’s limited to her intestine and lymph nodes only. Spleen shows hematopoiesis (blood cell production…it’s not a normal site for this process), but her blood test shows hemolysis (breakdown of red blood cells). Likely that the spleen has gone in, as bone marrow isn’t producing enough…this is called Extramedullary Hematopoiesis…so her cause will also need to be looked at.

We have the option of Chemotherapy (with steroids)…once in remission, she will stay on it for life, and to ensure lymphoma doesn’t return. Downside…it weakens the heart and she already has HCM.

I wish I had better news. I wish I had said it was IBD…I wish i was told that’s all it was, or food sensitivity…but for now, here she is…in her suit. She’s not grumpy, she’s not in pain…she’s just sleepy on her Gabapeptin for the post surgical pain relief.

Sorry we’ve all been quiet. We’ve had a lot going on with the family…and now this.

So allow me to help you catch up.
Luna attended Snoots today for her pre op blood tests. Why? Well…Luna was diagnosed with IBD 7 years ago (following biopsies) and post treatment, she has been as good as gold. Then recently, weight loss happened despite no change in behaviour or appetite. She had a check, was all ok…then, diarrhoea started. Some things worked (temporary relief), some things didn’t (that should have worked).
Until one day…it was red. Watery. Frank blood diarrhoea. And then within 6 hours, vomit with frank blood (red, few coffee grounds) and a lot of it.
Snoots were quick enough to see her, loaded her up on supportive meds and referred her to ElizaVet for an ultrasound and X-rays.

The inevitable…enlarged, hyperchromic and solid lymph nodes, thickened intestinal lining. With my medical background, I knew what it looked like. Or at least where it was pointing to. And we dove in, discussing Small Cell Carcinoma (did you know IBD can progress to SCC? You do now), potential Chemo vs Palliative, as it’s also damaging on the heart (it’s a case of “do you treat one evil, to worsen the other evil”)…but first…surgery to determine what’s happening.

This was the first few weeks when she’s had diarrhoea that couldn’t be resolved. She’s had a handful of episodes in the last 7 years…nothing that wasn’t resolved within 48 hours. This, this was horrendous.

We opted for an open surgery vs endoscopic procedure. Yes, endoscopic offers quicker recovery and is less invasive but it comes with its own limitations (especially manoeuvring the organs, visibility, and it’s slightly longer than open surgery as well). She will have biopsies of the lymph nodes, intestine, as well her liver and spleen (for staging if it is SCC - best to do it all at once than go in again).
Currently…we’re working on the middle ground scenario that it is SCC. Worst case is Large Cell Carcinoma, and the prognosis is worse…best would be IDB of course, but we can only hope.
But medically speaking, there are red flags which cannot be ignored. Her weight loss being a main one.

So, here she is…having a little hump over bloods being taken. For which she was very good and very patient with the vet, and me holding her paws to put some pressure on the vein.

This flower is maturing well... This week for the first time, she's been learning to master the cat wheel... And in her true (not so) Blossom fashion, this also meant that she ensured that everyone knew it was HER turn.

You may look at this cute little girl and think "delicate little flower"...that was the idea. I prayed she would grow into her name. And she did... Almost. Most of the time, she's daddy's shadow. Where dad is, she is next to him. Mummy gets cuddles in the morning. Any Flora, is someone you rough play with. But...With other cats in the house, they all avoid her. When she's awake, everyone sleeps tucked away and hidden the best that they can because this duracel powered thorn will find them, pounce on them and then bounce on them. She terrorises them to get them to chase her. It's her way of wanting to play. They say that cats sleep for 18 to 20 hours per day... This child only sleeps for 8 hours at night and 2 during the day after breakfast. Kai.. The little Russian Toy terrier, who is no bigger than her... She torments him too. She use to wack him, then hide behind mums trousers and then mum will step in to show him who's the boss for upsetting her daughter... And she still continues, with or without mum's help. Kai doesn't bite...like many dogs, small dogs... He snaps and squeels... if she sits with you, that means that the radius of 1meter is all hers. Find another way to pass. And if anyone is brave enough to as much as cross it... She chases. But... She stopped beating me when I kiss her. So I guess that's a bonus lol

Happy Monday from Chonky Mouse Myshka, and Fullumptous Flo Flo

Whilst Myshkas weight (a tender 4.8kg) is not healthy, which we are aware off...she has been this weight since she was pregnant.

What does she eat you ask? Raw. How much of it? A lot less than you think!

So why? Who knows...however, she has lost 200g since being on her new diet.
You see, for some raw works wonders...between 4 of my mums (moggy and 3 nakids) and 4 of mine...only two struggle to lose the weight with raw. Both Flora and Myshka have 40g of raw twice per day and still managed to gain weight vs 120g per day (plus 30g of oral biccies daily before bed) for the others.

So what's their diet? We've gone back to Hills Weight Management. This is not a direction of the vet...this is because previously, this particular food workes for us. And why? It's one of the few foods that contains L Carnatine which helps to transition fats.
I've never been an advocate for raw or for commercial - I've always been an advocate of "pick the food that works with the cat". Why? Raw isn't for everyone...as is commercial isn't for everyone. There are risks with both - we've lived through them, so it's always important to understand those risks.
And risks aside, some...like my two girls...simply will continue to gain.

So for now...both Flora (who went down to 3.2kg and then back up to 5.1kg) and Myshka (who is supposed to be 3.2kg as well) are on a strict diet. They have 30g of raw and 10g of Hills Weight Management twice per day, and another 5-10g of Hills at night.
