Equine Gastric Ulcer Syndrome is a condition that can affect horses of all ages. Horses are particularly prone to gastric ulcers due to their unique digestive anatomy and physiology. Key contributing factors include:

1. Feeding Practices:
🐴 Horses continuously produce stomach acid, even when not eating.
🐴 Infrequent feeding or long periods without forage leave the stomach lining exposed to acid.
🐴 High-grain/low-forage diets increase acidity.
2. Exercise
🐴 Intense exercise increases abdominal pressure, forcing acid to splash onto the squamous portion of the stomach.
3. Stress
🐴 Transport, competition, confinement, or social stress can contribute to ulcer development.
4. Medications
🐴 Non-steroidal anti-inflammatory drugs (NSAIDs) like phenylbutazone can reduce protective mucus in the stomach.
5. Foals
🐴 Foals may develop ulcers due to stress, infrequent nursing, or illness.

The Pain Cycle, let’s break it down!

1. Pain: It all starts with a painful stimulus, like an injury or underlying health issue.

2. Muscle guarding: In response to pain, the muscles around the affected area tense up or guard, trying to protect the body.

3. Inflammation: The body’s natural response to injury or pain includes inflammation, which can cause swelling and heat in the affected area.

4. Decreased function: Pain and inflammation can lead to a decrease in the animal’s performance and signs of lameness.

5. Compensation: To cope with the discomfort, horses and dogs may adjust their movement or posture, compensating for the pain in other areas of the body.

Physiotherapy can help break the pain cycle by addressing the underlying causes of pain and be proactive in promoting healing. Through pain management techniques, muscle strengthening exercises, and rehabilitation, physiotherapy works to alleviate discomfort, restore function, and prevent the pain cycle from continuing.

** Stringhalt **

There have been a large number of cases of stringhalt recently.

Stringhalt is caused by pastures containing certain plants known to be toxic to horses, such as (Hypochaeris radicata), also known as flatweed or false dandelion.

Signs of stringholt include abnormal gait and involuntary over flexion of the hind leg, often so extreme it looks like the horse is kicking their stomach. It can sometimes also be seen when the horse is walked backwards or in a tight circle.

Once a horse has stringhalt, there are various things that can be done. Recovery time can vary from days to years.

The following should be done in consultation with your vet and horse's bodyworker:

👍🏻 Talk to your horse's vet. There are medications that have been shown to improve stringhalt

👍🏻 Book a bodywork session to help your horse. The methods I use are gentle and help the horse feel better. Additional tools, such as my PEMF machine that targets specific areas and can pe*****te quite deeply, can help reduce inflammation and keep nerves healthy.

👍🏻 Nutrition - check your horse's diet. Adding a toxin binder, extra magnesium and vitamin E can be helpful, but you do want to be aware of what you are currently feeding. Vitamin E should be natural and not synthetic. A good vitamin and mineral product is also highly recommended. There are also various herbal supplements that could help.

👍🏻 Rehab - exercises to help your horse can be really beneficial, but ensure they are tailored to your horse. Some exercises prescribed traditionally are not suitable and are too much for the horse at its particular stage of recovery.

I have a few clients I'm helping with stringhalt recovery and it's going well as we have addressed all of the above. It's a tough time. Having had a horse that got stringhalt, I know how hard it is, but we can get through this!

X-rays and scans show the status of the tissues being imaged BUT they don’t measure pain.

Horses can, same as people, have severe changes but absolutely no pain or conversely have minor changes sadly suffering significant pain.

We need to be aware of any signs of pain but also be aware that there is not automatically pain, as the changes can be incidental findings.

I had two cases today demonstrating this point. One with severe osteoarthritis of the hind fetlocks and one with very advanced kissing spines. If you over-read their x-rays you’d have thought the pain would have been extreme and end of the road for them. In both horses the severity of the changes in the x-rays were a complete surprise to the vets and the owners.

Future management plans and training expectations have been modified as a result of the findings. However both horses are staying in work with very caring owners and an a multi-disciplinary team supporting them.

At the moment the plan is to ride the horse in front of them each day, look out for but don’t anticipate clinical signs that are not there.

If your horse has a diagnosis, remember to care for them as horse first, pathology/injury second and take each day at a time.
