📍Case: A Jersey cow recently parturited was presented with history of injury by neighbouring cow leading to laceration of teat.
📍Clinical finding:
✔Full thickness vertical laceration leading to formation of teat fistula with leakage of milk.
📍Surgical procedure:
1.Ring block was given at the base of teat with Lignocaine 2%
2.Wound freshened n copiously washed with antiseptic solution.
3.Insertion of baby feeding tube no 8 to keep patency of the teat canal.
4.The wound closed in 3 layers (mucosa - submucosa, muscles, skin) using Vicryl 3-0.
5.ASD done with bandaging of the wound keeping the baby feeding tube in situ.

✔Broad spectrum antibiotic
✔NSAIDs
✔Multivitamins

📌Management:
📍Surgical debridement of the wound
📍Removal of granulomatous mass from teat canal using teat tumour extractor.
📍Fixation of baby feeding tube
📍Broad spectrum antibiotic, NSAIDS, local application of povidone iodine.
📌Complications:
📍Mastitis ( Encountered by use of antibiotics)
📍Failure of retention of baby feeding tube ( Replaced the baby feeding tube by foley’s catheter)
📌Result: Teat end wound healed at day 30.

“Management of teat obstruction in HFCB Cow due to teat end injury”
✅This cow had history of teat end injury due to self trauma which lead to the obstruction at its teat end.
✅After proper restraining and under aseptic conditions the growth was removed using teat tumour extractor followed by insertion and fixing of baby feeding tube to keep teat canal patent.
✅Follow up treatment continued with broad spectrum antibiotic and NSAIDS.

𝐒𝐮𝐫𝐠𝐢𝐜𝐚𝐥 𝐦𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭 𝐨𝐟 𝐦𝐚𝐦𝐦𝐚𝐫𝐲 𝐭𝐮𝐦𝐨𝐮𝐫 𝐢𝐧 𝐚 𝐝𝐨𝐠
➡️Surgical technique: Mammectomy was performed under GA.
📌Mammectomy is a removal of a single mammary gland, including the ni**le and skin overlying the gland.
📌It is a good choice when the mammary mass is directly under the ni**le or is fixed to the overlying skin.
📌It should not be used for masses fixed to the underlying re**us fascia.
📌In this case the mammary tissue was superficial to the ventral fascia of the re**us muscle and typically has a layer of fat directly under the glands.
➡️Post op : Fluid therapy, broad spectrum antibiotics, Nsaids and multivitamins.
➡️Surgical pearl : During caudal dissection, ligate the caudal superficial epigastric artery and vein to prevent excessive bleeding.

🐶𝐌𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭 𝐨𝐟 𝐇𝐞𝐦𝐚𝐭𝐨𝐦𝐚 𝐨𝐧 𝐂𝐡𝐞𝐬𝐭 𝐰𝐚𝐥𝐥 𝐨𝐟 𝐃𝐨𝐠🐶
📌A non descript dog was presented with history of automobile accident since 4 days with development of swelling around left side of the chest.
📌Clinical examination revealed that the swelling was non painful, soft and fluctuating with presence of fluid thrill.
📌Needle aspiration revealed blood tinged fluid , thus found to be a Hematoma.
📌Management: The swelling was lenced at the dependent part with removal of approx. 4 to 5 lts of blood tinged fluid and fibrin clots. The cavity was thoroughly flushed with antiseptic solution followed by pressure bandage.
📌The patient was stabilised with fluid therapy, broad spectrum antibiotic,NSAIDS and multivitamins.
📌The patient recovered uneventfully and relocated on 5th day.

Clinical examination revealed that the mass was soiled, edematous, lacerated and necrotic.

Hysterectomy was performed under GA after stabilisation of the patient.

🎗Manegement: Full limb PVC splint POP cast was applied under sedation.
🎗Important steps:
✔ Sedation with Xylazine
✔ Traction and counter traction for proper alignment of the proximal and distal frature ends
✔ Cotton padding followed by bandaging
✔ Application of PVC splints
✔Application of POP cast.
✔NSAIDs and multivitamins for 3 to 5 days.

💕The flap was removed on 15th day and
Eventually the eye appears clear with a white scar on day 60💕

❎A case was presented with history of chronic Cervicova**nal prolapse since 6 months.
❎The prolapse was corrected manually earlier 4 to 5 times by the conservative methods like by using rope truss, buhner sutures, medicinal treatment etc etc but it recurred.
❎Thus the treatment was done by applying button suture using Modified Minchev's Vaginopexy technique.
❎Procedure
1. Epidural local anesthesia
2. Plastic buttons, nylon/ cotton thread, long cutting needle.
3. Cleaning and disinfecting the pr*****ed mass, emptying of urinary bladder followed by reduction and repositing of it.
4. The threaded needle with placed button is passed from the va**nal cavity to outside through the gluteal region, posterior to sacrosciatic ligament.
5. Then 2nd button is fixed outside at the point from where the needle comes out.
6. Both the buttons are fixed tightly by tying the knot out side the va**na.
7. Same is done on other side.

➡️A case was presented with history of swelling at the lateral aspect of neck in horse since one month ,after it got some IM injections.
➡️Clinical examination finding were
✔Temperature 101*f.
✔The swelling was warm, painful and hard (++) but soft at few spots.
✔Needle aspiration revealed only a small amount (0.5ml) of thick serosanguinous contents.
✔Based on history and physical examination, it was diagnosed as injection site abscess
➡️Treatment
✔Standing sedation was given followed by local infiltration of lignocaine 2% at the incision site was given.
✔Firstly subcutaneous stab incision was given but nothing came out and thus on further exploration it was found that the abscess was deep inside the muscles which has led to the myonecrosis cm myositis.
✔Then it was decided to perform the Fasciotomy.
✔After which thick insippated pus along with necrotic muscles were removed.
✔The cavity was thorougly flushed with jet of fresh water followed by flushing the cavity with Povidine iodine solution.
✔Later the cavity was packed with antiseptic gauze.
➡️Post op
✔Broad spectrum antibiotic
✔NSAIDs
✔Tetanus toxoid
✔The gauze was changed on alternate days
➡️Result: The would completely closed on 21st day.
𝐅𝐚𝐬𝐜𝐢𝐨𝐭𝐨𝐦𝐲
It is a surgical procedure in which the skin, sub-cutaneous tissue, and fascia are incised for the purposes of drainage and/or decompression of deeper tissues.
Procedure
• Clip hair and prepare skin with aseptic technique.
• Inject local anesthetic into the subcutaneous tissues.
• Incise the skin over the area to be drained or decompressed. Incisions should be at least 1 cm in length. If the affected area is extensive, lengthen the incision(s) parallel to the lines of maximal resting skin tension (if possible) to afford the necessary drainage or decompression.
• If treating infection, copiously lavage the incised tissues.
• Cover fasciotomy sites with bandage material to absorb drainage and minimize gross contamination.
• Allow the incision(s) to heal by second intention.

✔Case 1 This heifer had a chronic non healing wound at the distal end of the forelimb since many day
✔The wound was thorougly cleaned and lavaged with antiseptic solution
✔Chemical debridement using CuSo4 dressing was done on alternate days for 3 times followed by routine dressing with corticosteroids
✔The wound healed successfully on 40th day
❎The main reason for formation of EGT is weak, delayed and protracted inflammatory response of the body towards wound, which is most commonly found in equines
❎So, when treating wounds by second intention, inflammation should be stimulated until wound has filled with granulation tissue and there after it should inhibited

✔This type of closure occurs when a wound heals on its own by forming granulation tissue, epithelializing, and contracting.
There are three primary phases of secondary intention healing:
➡️Filling the wound – New tissue, known as granulation tissue, will fill the base of the sore and create new blood vessels.
➡️Contraction – As the body creates new layers of skin, the outer edges of the wound pull towards the center of the exposed area, slowly closing it up.
➡️Re-epithelization – Epithelial cells emerge from the margins of the wound to cover the area with a new layer of healthy skin. At first, the skin will be soft, thin, and more delicate, but will thicken and strengthen over time.

❎Case: A Jersey cow was presented with history of barbed wire injury leading to laceration of teat.
❎Clinical finding:
✔Full thickness vertical laceration leading to formation of teat fistula with leakage of milk.
❎Surgical procedure:
1.Ring block was given at the base of teat with Lignocaine 2%
2.Wound freshened n copiously washed with antiseptic solution.
3.Insertion of baby feeding tube no 8 to keep patency of the teat canal.
4.The wound closed in 3 layers (mucosa - submucosa, muscles, skin) using Vicryl 3-0.
5.ASD done with bandaging of the wound keeping the baby feeding tube in situ.

✔Broad spectrum antibiotic
✔NSAIDs
✔Multivitamins

❎History:
✔The owner of this cow was not known about her pregnancy status, however she came to heat and got naturally inseminated.
✔On 2nd day he found serosanguinous va**nal discharge.
❎Physical examination and management:
✔On PR examination a hard mass was felt at the base of horn.
✔Then PV was done and a mummified fetus was removed manually.
✔The gestational age of the fetus (approx 100 days) was assessed by measuring its crown-rump length.
✔Broad broad spectrum, oral and intra uterine ecbolic was given.

❎The umblical abscess was confirmed based on physical examination a d needle aspiration.
❎Treatment
✔A stab incision was given on the most dependent part followed by evacuation of the pus.
✔The cavity was explored thoroughly for any foreign body.
✔Later, the cavity was tightly packed with Tincture iodine soaked gauze.
❎Follow up
✔Broad spectrum antibiotic
✔NSAIDs
✔Change of gauze for 2 to 3 days.

🎗Surgical procedure: Amputation of the tail
🎗Technique:
✔Anesthesia: G.A & epidural local anesthesia
✔Application of tourniquet
Making two V-shaped skin incisions that meet to make a dorsal and ventral skin flap
✔Dissection of subcut tissue and muscles
✔Ligation of the lateral and median vascular bundles
✔Transection of the tail via disarticulation between vertebral segments
✔Closure of soft tissue and skin over the exposed and remaining caudal vertebra
🎗Post op :
Antibiotic, NSAIDs, ASD, Gastorprotectant, Multivitamins.

🎗Manegement: Full limb PVC splint POP cast was applied under mild sedation.
🎗Important steps:
✔Mild sedation with Xylazine
✔ Traction and counter traction for proper alignment of the proximal and distal frature ends
✔ Cotton padding followed by bandaging
✔ Application of PVC splints
✔Application of POP cast.
✔NSAIDs and multivitamins for 3 to 5 days.
