Another extraordinary success story for Dr. Omar Del Vecchio and his team!

Egly, a 6-year-old female Amstaff, was experiencing hind limb mobility difficulties with a complex neurological condition. Thanks to an accurate diagnosis and a targeted surgical procedure involving a mini-hemilaminectomy and vertebral stabilization, Egly was able to regain mobility and significantly improve her quality of life.

➡️Reported symptoms and reason for examination: For some time, she has had difficulty with hind limb mobility, more pronounced in the right hind limb.

➡️Neurological examination: Ambulatory paraparesis, associated with bilateral proprioceptive deficit in the hind limbs, more marked on the left side. Spinal reflexes are normal. Pain upon palpation of the thoracolumbar spine, with “crepitus” detected at the junction.

➡️Neurological diagnosis and comments: Protrusion-extrusion at T13-L1 and L5-L6, both lateralized to the left. Left-sided mini-hemilaminectomy at L5-L6 and T13-L1 with vertebral stabilization and distraction using a left-sided Pedro system at T13-L1 (2.7 screws and 4 cm rod).

WHWT (West Highland White Terrier), Rudy, Male, 11 years old.

➡️ Reported symptoms and reason for the examination: For some time, he has been experiencing difficulty with posterior locomotion (hind legs).

➡️Neurological examination: Ambulatory paraparesis, associated with bilateral proprioceptive deficits in the posterior limbs, more pronounced on the right side. Spinal reflexes are normal. No pain was elicited upon palpation of the thoracolumbar spine.

➡️ Diagnosis and neurological comment: Right dorsolateral subarachnoid diverticulum at T9-T10, with disc protrusion at T8-T9 and T9-T10, treated with right hemilaminectomy, durotomy with removal of the diverticulum, followed by dural defect repair. Vertebral stabilization and distraction using a Pedro monolateral system on the right side of T8-T9-T10 (2.7 mm screws and a 6 cm rod).

In this case, we present the story of Lupa, an 8-year-old German Shepherd with paraplegia and reduced pain sensitivity.

The patient's radiographic examination revealed the outcome of the consolidation of a previous L4 fracture with deviation and stenosis of the vertebral canal at the L3-L4 level. Additionally, widespread idiopathic skeletal hyperostosis was noted in the vertebral segment caudally to the lesion.

Dr. Raffaele Gilardini, neurologist and neurosurgeon, then performed surgery using the Pedro System designed by Overvet.

Let's share the story of Luna, a 4-year-old, 37 kg German Shepherd.

Over the past three months, Luna has experienced episodes of intense pain during physical activity, immobilizing her for several minutes and eventually leading to lower back pain.

After thorough examinations, Luna was diagnosed with a lumbosacral transitional vertebra (LSTV) and degenerative lumbosacral stenosis secondary to L7-S1 disc protrusion in the epidural region. This condition is associated with vertebral subluxation and compression of the cauda equina nerve roots.

It was decided to proceed with surgery to relieve her pain and improve her quality of life. Luna underwent a dorsal L7-S1 laminectomy, L7-S1 discectomy, and arthrodesis, expertly performed by Dr. Cristian Falzone, using the Pedro System spinal fixation and distraction system developed by Overvet.

➡ Breed: Mixed-breed dog, weight 30 kg, female, 5 years old.

➡ Symptoms and relevant history: Trauma from being hit by a car. Lumbosacral fracture with vertebral dislocation and pneumothorax. Referred to NVA clinic the following week for surgical therapy.

➡ X-ray diagnosis: Lumbosacral dislocation and fracture of the sacral arch in S1 with involvement of the sacral vertebral canal by the body of L7, dorsally dislocated.

➡ Surgery: Reduction and stabilization. Insertion of two transarticular screws in the lumbosacral articular facets after reduction of the vertebral dislocation; application of the PedroSystem with two sacral transpedicular screws 4 mm in diameter and two pedicular screws 3.5 mm in diameter in L7.

Mixed-breed dog, male, 9 years old, weight 16 kg

➡️Clinical signs: Lumbosacral pain and mild right rear limb lameness.

➡️Diagnosis: Degenerative lumbosacral stenosis (DLSS) characterized by disc herniation and vertebral instability at L6-L7. A transitional L7 with sacralization is evident. L7 has the morphology of the first sacral vertebra.

➡️Surgery: Reduction of articular facets with the application of two transarticular screws. Removal of extruded disc material. Removal of dorsal disc ring and right paramedian herniated portion. Application of PedroSystem polyaxial screws Ø2.7mm: 2 screws in sacralized L7 and 2 screws in L6.

➡️ Clinical signs:

Pain when getting up and stiffness in the hindquarters.

Degenerative lumbosacral stenosis with marked spondylosis, reduced intervertebral space, L7-S1 disc protrusion in the epidural area, resulting in marked compression of the cauda equina nerve roots, along with concomitant mild vertebral instability.

➡️ Surgery:

Dorsal laminectomy L7-S1, discectomy L7-S1, and arthrodesis.

➡️ Follow up:

Good clinical condition, implant in place.

➡️ Follow up at 2 months: Good clinical condition, implant in place, distraction well maintained.

It is not only an innovation in veterinary surgery, but above all an act of civilization towards the animal world, allowing our 4-legged friends to return to their lives! This is the goal in designing our spinal fixation and distraction system to be used in lumbosacral stabilization. In the video, we see an 8-year-old Labrador retriever in full recovery after 3 hours from the surgery. Here are the details: ➡️ CLINICAL SIGNS: Relapsing low back pain without response to medical therapies. ➡️ RADIOGRAPHIC DIAGNOSIS: mild spondylolisthesis and spondylosis L7-S1 with increased radiopacity of formina. ➡️ MRI DIAGNOSIS: severe L7/S1 disc protrusion with compression of the cauda equina nerve roots, bilateral foraminal stenosis and ventral spondylosis. The resonance in traction highlights a lesion of a dynamic type (lumbo-sacral instability). ➡️ SURGERY: L7-S1 laminectomy, dorsal fenestration of the disc and removal of the disc protrusion, distraction and insertion of a cage into the disc space, L7/S1 stabilization with 2.4 mm titanium transarticular screws and 3.5 mm polyaxial pedicle screws. ➡️ POSTOPERATIVE RADIOGRAPHY: good L7-S1 alignment and vertebral distraction, cage positioned correctly, with opening of the foramina. ➡️ POSTOPERATIVE PROCESS: The patient gets up 3 hours after surgery and is discharged after 24 hours.
