Today, we are excited to share a new success story!

Dr. Falzone performed surgery on Tiago, a Border Collie, who was suffering from vertebral luxation at the T13-L1 level secondary to discospondylitis.

For the procedure, the Pedro System, an advanced spinal distraction and stabilization system, was used to ensure the best possible recovery for the patient.

The Pedro System is an innovative surgical device for spinal fixation, specifically designed for veterinary use. This system, composed of polyaxial screws, rods, and cages, offers a wide range of sizes (from 2mm to 4.5mm), making it a comprehensive solution for spinal stabilization. Thanks to its flexibility and ease of use, it represents an excellent alternative to the use of pins and PMMA. Additionally, the nuvaBONE nano-structured biomimetic hydroxyapatite biomaterial enhances the procedure in cases of bone deficiency, promoting better healing.

The choice of which spinal system to develop first fell on the larger one. Subsequently, we worked on the smaller one, allowing us to have the two extreme cases for mechanical stress testing.

After a few weeks of work in the workshop, the components were finally ready. Eager to verify the accuracy of the measurements derived from our initial studies, the first thing we did was assemble our newly created "Pedro System" onto a canine spinal section made of plastic material. At first glance, everything seemed to function as expected: the length and diameter of the screws, the interaction between the various mechanical components—every detail appeared perfect.

At that point, all that remained was to produce the smaller spinal system, which, due to its reduced size, was logically more prone to potential mechanical issues. Its development was relatively quick, and within a few weeks, we were ready to take the components to the laboratory for mechanical stress testing.

The tests conducted later at the Politecnico di Milano would provide the definitive answer: had months of work been well spent?

Today, we’re sharing the story of a special patient: a 10-year-old Dalmatian weighing 29.7 kg, presenting with lumbosacral (LS) pain, dorsal disc protrusion at L7-S1 and bilateral foraminal stenosis.

Clinical presentation:

✔️ Significant pain

✔️ Reluctance to engage in certain daily activities

✔️ Right-sided nerve root signature, associated with foraminal stenosis

Thanks to the expertise and collaboration of Dr. Lucas Smolders and Dr. Niklas Bergknut, and the use of the groundbreaking PEDRO System, this complex case was managed with a targeted and effective surgical approach.

After several hours of intensive design work, the first draft of the Pedro System, an innovative spinal system, is finally ready. All the components that make up the system have been meticulously modeled and displayed on a monitor:

✅ Polyaxial pedicle screws
✅ Set screws for rod fixation
✅ Spacer cages, designed to be manufactured using 3D printing technology.

At this stage, the next step is to transform this digital design into a real and tangible spinal system.

Production and Collaboration:

✅ Files for the components produced using additive manufacturing technology were sent to highly specialized external suppliers.
✅ The remaining components were produced in-house through our subsidiary, Leghe Leggere Lavorate, a company with over 25 years of experience in manufacturing implantable medical devices for both human and veterinary markets.

The close collaboration between designers and production managers ensures:

- Resolution of potential production issues, improving the quality and performance of the components.

- The implementation of strategic modifications, informed by the extensive experience of our production partners, leading to enhanced technical features and cost reductions.

This project is a perfect example of how the synergy between engineering innovation and manufacturing expertise can deliver exceptional results in the field of medical devices.

Clinical Case Spotlight: Chihuahua Dog - Lucki, Male, 8 years Old.

➡️ Symptoms and Examination:
Lucki presented with recent difficulty walking in his hind legs, accompanied by kyphosis (arched back) and severe pain on palpation of the thoracolumbar spine. During the neurological examination, he exhibited:
- Mild ambulatory paraparesis
- Bilateral proprioceptive deficits on both hind limbs.
- Normal spinal reflexes
- Severe pain localized to the thoracolumbar spine

➡️Surgical Treatment:
right T12-T13 mini-hemilaminectomy with removal of herniated disc material, fenestration of the intervertebral disc, and unilateral vertebral stabilization and distraction (T12-T13) using the Pedro system (2.4 mm screws and a 3 cm rod).

This complicated procedure was performed by Dr. Omar Del Vecchio, whose expertise in veterinary neurology and spinal surgery has made a remarkable difference in Lucki’s life.

➡️Outcome and Hope:

Veterinary Excellence: a case of lumbosacral stenosis successfully treated by Dr. Omar Del Vecchio

➡️Patient: Maremmano, Ax, male, 7 years old
➡️Symptoms and reason for the exam: Recent difficulty standing with the hind legs, sometimes accompanied by spontaneous yelping.
➡️Neurological examination: Hind limbs positioned "underneath" the body, associated with a reduced withdrawal reflex in the left hind limb and severe pain during flexion and extension of the lumbosacral spine.
➡️Final diagnosis and MRI findings were consistent with degenerative lumbosacral stenosis, secondary to L7-S1 disc protrusion, resulting in compression of the cauda equina roots.
➡️Surgical treatment: L7-S1 dorsal laminectomy with removal of the disc protrusion, application of a customized PMMA prosthesis to correct the resulting bone defect. Stabilization in distraction using polyaxial screws (3.5 mm diameter) and fixation of the articular facets with titanium screws (2.7 mm diameter).

A remarkable achievement in improving the patient’s quality of life!

In the video we share today, we tell the story of Rudy, a West Highland White Terrier with severe motor difficulties in his hind limbs due to a malformation in one of the meninges surrounding the spinal cord (subarachnoid diverticulum).

To help him, Dr. Omar Del Vecchio of the Caleidos Veterinary Center and his team performed surgery to remove the diverticulum and stabilize the area using right-sided unilateral vertebral distraction of the T8, T9, and T10 vertebrae with our innovative Pedro System.

The Pedro System continues to demonstrate its potential to improve the quality of life for our four-legged friends, enabling effective and minimally invasive surgical interventions.

Pedro System: The Initial Pre-Project Evaluations

As mentioned in previous posts, the development of our Pedro System stems from the experience gained over the years with devices dedicated to human spinal surgery.

The idea was to leverage the design features of these devices, reducing their size to be compatible with the canine skeletal structure.

To achieve this, we had to first evaluate the vertebral bone dimensions across different dog breeds, as they vary significantly in both size and weight.

This led us to the development of a product with various sizes that would be compatible with the majority of potential patients.

After gathering the necessary information, we began assessing how many sizes would be needed to cover the entire range of spinal bone structures that surgeons could encounter during operations.

This resulted in the creation of three different spinal system sizes: one for small dogs (7 kg), one for medium-sized dogs (38 kg), and one for large dogs (70 kg).

Case Study: A Journey to Recovery for Nina, Our Resilient Pointer

Today, I’d like to share the story of Nina, a 1.5-year-old female pointer who faced a challenging neurological condition affecting her quality of life. Adopted from a breeder, Nina had recurring episodes of sudden pain, limb stiffness, yelping, and difficulty standing after rest. Her neurological examination revealed moderate ataxia in all four limbs and significant cervical rigidity, especially during neck movement.

➡️ Diagnosis and Surgical Intervention
Imaging tests, including X-rays, MRI, and a CT scan, confirmed agenesis of the dens of the axis, leading to atlanto-axial instability – a painful condition causing her distress. After careful assessment, Nina was selected for atlanto-axial stabilization surgery. Dr. Raffaele Gilardini skillfully performed the procedure, placing titanium screws and OVERMED implants to secure vertebral stability and grafting spongy bone from her humerus.

➡️ Recovery and Follow-Up
Just 24 hours post-surgery, Nina was discharged, walking independently, and showing notable improvement in cervical pain control. Ten days later, she’s progressing well, with no further cervical pain and newfound ease in getting up. While mild cervical stiffness remains, her resilience shines as she continues on her journey to full recovery.

Let’s discover the results of the last spine stabilization performed by Dr. Omar Del Vecchio of Centro Veterinario Caleidos.

WHWT (West Highland White Terrier), Rudy, Male, 11 years old.

➡️ Reported symptoms and reason for the examination: For some time, he has been experiencing difficulty with posterior locomotion (hind legs).

➡️Neurological examination: Ambulatory paraparesis, associated with bilateral proprioceptive deficits in the posterior limbs, more pronounced on the right side. Spinal reflexes are normal. No pain was elicited upon palpation of the thoracolumbar spine.

➡️ Diagnosis and neurological comment: Right dorsolateral subarachnoid diverticulum at T9-T10, with disc protrusion at T8-T9 and T9-T10, treated with right hemilaminectomy, durotomy with removal of the diverticulum, followed by dural defect repair. Vertebral stabilization and distraction using a Pedro monolateral system on the right side of T8-T9-T10 (2.7 mm screws and a 6 cm rod).
