L’uso della fissazione esterna come adiuvante nelle fratture periarticolari può essere particolarmente vantaggioso in tutte quelle situazioni in cui si desideri mantenere la mobilità articolare, assicurando nel contempo la stabilità della frattura che, per ragioni dimensionali dei frammenti coinvolti, è spesso stabilizzata con tecnica a minima. In questi casi, infatti, con la sola osteosintesi interna il braccio di leva che si riesce a esercitare è sempre inferiore rispetto a quello esercitato dai carichi durante la deambulazione, per cui il rischio della mobilizzazione degli impianti è molto alto se viene concesso il carico completo. Con l’utilizzo della fissazione esterna, invece, possiamo contrastare le forze di carico con un braccio di leva di entità molto maggiore, proteggendo l’osteosintesi interna, ma consentendo nello stesso tempo la mobilità articolare, fondamentale per non incorrere in una anchilosi da immobilizzazione prolungata.

Use of the trans-articular articulated fixator for the treatment of peri- and juxta-articular fractures
The use of external fixation as an adjuvant in periarticular fractures can be particularly advantageous in all those situations in which it is desired to maintain joint mobility, while ensuring the stability of the fracture which, due to the size of the fragments involved, is often stabilized with a weak osteosynthesis. In these cases, in fact, with internal osteosynthesis alone, the lever arm that can be exercised is always lower than that exerted by loads during walking, so the risk of implant mobilization is very high if the full load is allowed. With external fixation, on the other hand, we can counteract the load forces with a much greater lever arm, protecting internal osteosynthesis while allowing joint mobility, which is essential to avoid ankylosis due to prolonged immobilization

Immagini pre e post operatorie del trattamento di una frattura distale di radio ulna mediante la filosofia della osteosintesi ibrida interna/esterna. Questa filosofia permette di effettuare un’osteosintesi interna con impianti di piccole dimensioni e un accesso minimamente invasivo, utilizzando il fissatore esterno che ha la funzione di proteggere l’impianto interno grazie al braccio di leva molto più potente. Il fissatore viene ovviamente rimosso una volta che si sia sviluppato un callo osseo soddisfacente. In questo modo vengono anche eliminate le possibili complicazioni legate alla fase di transizione dopo la rimozione del fissatore esterno senza impianti interni.
Il video mostra l’utilizzo dell’arto da parte del paziente due giorni dopo la chirurgia. Il recupero così rapido ovviamente favorisce la ripresa funzionale precoce dell’arto.

Pre- and post-operative images of the treatment of a distal radius ulna fracture using the philosophy of hybrid internal/external osteosynthesis. This philosophy allows internal osteosynthesis to be performed with small implants and minimally invasive approach, using the external fixator for protection of the internal implant thanks to the much more powerful lever arm. The fixator is obviously removed once a satisfactory bone callus has developed. This also eliminates possible complications related to the transition phase after the removal of the external fixator without internal implants.
