Surgical management of metacarpal fractures using plates & screws

The patient was a 15-yr old Italian greyhound with multiple metacarpal fractures secondary to car accident. They came all the way from Maramag Bukidnon & drive for almost 10 hrs going to Caraga Pet Doctors Butuan City for surgical correction.

Upon thorough examination, pre surgical lab screening, & careful consideration of the risk(since patient is already 15yrs old), the surgery was immediately performed & thankfully everything went well.

Days after the surgery, macaroni is starting slowly to use his affected leg & wound healing was great. Thanks to the efforts of his owners, watching closely their patient most of the time, religiously giving the medications & regularly giving us updates.

Perineal Hernia in Dogs

A few months ago, a 9-year-old aspin named Max was brought in with a large swelling near his rear. According to his owner, the issue has been ongoing for over a year. After a thorough physical examination and lab work, Max was diagnosed with perineal hernia. This condition occurs in dogs when the muscles of the pelvic diaphragm weaken or tear, causing abdominal contents to herniate into the perineal region (the area between the a**s and the sc***um or v***a). Perineal hernia is most commonly seen in older, unneutered male dogs, and early detection and treatment are crucial to avoid complications.

The exact cause of perineal hernias in dogs is not fully understood, but several factors may contribute to the weakening of the pelvic muscles:

• Hormonal Influences: Testosterone in intact males is believed to play a role, which is why neutering may reduce the risk.
• Chronic Straining: Conditions such as chronic constipation, diarrhea, or difficulty urinating can put strain on the perineal muscles, increasing the risk of a hernia.
• Age: The condition is most common in older dogs (typically over the age of 7).
• Breed Predisposition: Certain breeds, such as Boston Terriers, Welsh Corgis, and Boxers, are more prone to perineal hernias due to genetic predispositions.

Signs and Symptoms:
• Swelling near the a**s: The most common symptom is a visible or palpable swelling in the perineal region. It may be soft or firm and can vary in size.
• Difficulty defecating: The hernia may obstruct the re**um, leading to constipation or straining during defecation.
• Urinary issues: If the bladder becomes trapped in the hernia, dogs may experience difficulty urinating, frequent urination, or incontinence.
•Pain or discomfort: Some dogs may show signs of discomfort, restlessness, or pain, especially when trying to defecate or urinate.
• Lethargy or loss of appetite: If the hernia becomes severe or strangulates an organ, the dog may become lethargic, refuse food, or show other signs of systemic illness.

Treatment: Surgical correction is the most effective treatment for perineal hernia. The goal of surgery is to repair the weakened or torn muscles and, if necessary, reposition any herniated organs.

Postoperative Care: Postoperative care is crucial for a successful outcome. Dogs may need pain relief, antibiotics, and stool softeners to reduce straining during recovery. Restricted activity is important to allow the surgical site to heal, and the patient will need to wear an e-collar to prevent licking or biting at the wound.

After further discussion with Max's owner, the decision was made to proceed with surgery to correct the herniation. Following the completion of pre-surgical lab work, Max was cleared for the operation, which fortunately went smoothly and free from complications. He was also neutered to minimize the risk of recurrence due to hormonal factors. Max was advised to remain at the clinic for just over a week for continued monitoring and to ensure proper postoperative care. Months later, we are pleased to report that Max is now living a normal life. He has gained weight since he can now eat and defecate without issues, and he appears much happier than when he was first brought in for evaluation. 🐶

Gastric Dilatation and Volvulus in Dogs

Our humble practice recently treated a dog with this specific condition last August 20, 2024. The patient came in for x-ray referral due to possible ingestion of foreign material 24hrs ago.

Symptoms and signs observed were unproductive retching, slouching, difficulty in breathing, delayed CRT, pale mm, hard and enlarged abdomen. Blood works were unremarkable.

On the right lateral view of the radiograph, compartmentalization of the stomach and dorsal displacement of the pyloric end was observed. This confirms that the dog has GDV, distention and subsequent twisting of the dog’s stomach.

GDV is an emergency and fatal situation as the twisting of the stomach can cause the following problems:

-cutting off the blood circulation of the stomach leading to necrosis
-splenic torsion
-stomach gas build-up leading to breathing difficulty
- Twisting that can lead to occlusion of the inferior vena cava leading to shock and eventually DEATH.

The patient was immediately taken in the OR and the following surgeries were performed:

-Gastrotomy (check for possible fbo and ingesta retrieval)
-Gastropexy (to prevent from re-twisting)
-Splenectomy (due to thrombosis)

The patient recovered well within 2 days and continue its medication in the comfort of its home.

This is Chimai, she was brought to us limping with its left pelvic limb after jumping from a chair.

Radiograph revealed an incomplete oblique fracture of distal tibia.

Our surgical team stabilize the fracture using a Dynamic Compression Plate and Independent Lag screws to prevent bone shearing.

24hrs after surgery and Chimai is already using the repaired leg.
