Librela, containing the monoclonal antibody bedinvetmab, is a new subcutaneous treatment for managing osteoarthritis pain in dogs. Clinical studies in the US and Europe demonstrated its effectiveness, with significant pain reduction observed after at least two doses administered one month apart. Success rates at Day 28 were 48% in the EU and 45.2% in the US, with the EU study showing a stronger effect. Continued treatment provided sustained pain relief through Day 84, making Librela a reliable option for enhancing the quality of life in dogs with osteoarthritis.

Know the Basics about Canine Babesiosis

• Canine Babesiosis is a disease caused by the protozoal parasites of the genus Babesia. Merozoites or piroplasms are the stage that infects the dog's red blood cells.
• Babesia gibsoni (Asia) is a small piroplasm that infects dogs, it has a worldwide distribution and an emerging disease in the Philippines. 8 out of 10 patients in the Philippines with clinical signs of anemia are positive for this blood parasite conducted via PCR for Babesia gibsoni.
• Infection may occur either by tick transmission, direct transmission via blood transfer during dog bites, blood transfusions or transplacental transmission.
• Incubation period averages about 2 weeks, but some are not clinically diagnosed for months to years.
• Babesia infects and replicates in the red blood cell, resulting in both direct and immune mediated hemolytic anemia.

Systems Affected

• Hemic/ Lymphatic/ Immune - anemia, thrombocytopenia ( bleeding tendencies appear rare), fever
• Splenomegaly
• Lymphadenomegaly
• Increased liver enzymes

Clinical Signs

• Signs can be peracute, acute ot chronic
• Some carriers have no detectable clinical signs
• Lethargy, anorexia, pale mucous membrane
• Icterus
• Weight loss
• Discolored stool

Predisposition/ Signalment

• Any age or breed of dog can be affected
• Babesia gibsoni (Asia) infections are more prevalent in American pit bull terriers.

Vet Diagnostix Babesia gibsoni Antibody Test kits- test for antibodies in the serum that reacts with Babesia organisms; cross reactive antibodies can prevent the differentiation of species and subspecies; some infected animals particularly young dogs and puppies may have no detectable antibodies.

Treatment

• May require inpatient or outpatient care, depending on severity of Babesiosis.
• Hypovolemic patients should receive aggressive fluid therapy.
• Severely anemic patients may require blood transfusion.

Drugs of Choice

1. Imidocarb dipropionate- 6.6mg/kg SC or IM every 1 to 2 weeks, Atropine sulfate 0.02mg/kg SC, 30 minutes prior to Imizol administration
2. Combination therapy of azithromycin (10mg/kg/PO q24hr for 10days) and atovaquone (13.5mg/kg PO q8h for 10 days) is the treatment of choice and the only treatment that can potentially clear B. gibsoni (Asia) infections in dogs. In a controlled study, 85% of dogs cleared the infection after treatment.
3. A combination of clindamycin (25mg/kg PO q12h), metronidazole (15mg/kg PO q12h) and doxycycline (5mg/kg PO q12h) for ellimination or reduction of the parasite 24 to 92 days.
4. A combination of doxycycline (7 to 10mg/kg PO q24hr), enrofloxacin (2 to 2.5mg/kg PO q12hr) and metronidazole (5 to 15mg/kg PO q12h) for 6 weeks.
5. For decreased Clinical signs but not to clear infection:
• Metronidazole (25 to 50mg/kg PO 24h for 7days)
• Clindamycin (12.5 to 25mg/kg PO q12h for 7 to 10days)
• Doxycycline (10mg/kg PO q12hr for 7 to 10days)

6. Prednisone 2.2mg/kg/day PO

Follow Ups

• Recheck the CBC and biochemistry as needed to monitor for resolution of anemia, thrombocytopenia, icterus and other signs.
• Most patients have a clinical response 1-2 weeks after treatment
• 2-3 consecutive negative PCR tests beginning 2 months post treatment should be performed to rule out treatment failure and persistent parasitemia.
• Vet Dx Babesia gibsoni antibody test are not recommended for follow up because titers may persist for years.
• When a patient housed in a multi-dog kennel is diagnosed with babesiosis, all dogs in that kennel should be screened since there is a high percentage of carrier animals in kennel situations.
• Co- infections with other vector transmitted pathogens like Canine Ehrlichia, Heartworm, Hepatozoon and Anaplasma should be considered, especially in patients that fail to respond to treatment.

Prevention
• Tick control. All attached ticks should be removed as soon as possible.
