LAKEWOOD BARK PARK CONTRACT
1. I wish to register to use the Lakewood Bark Park (hereinafter, the “Park”).
2. I understand that I must follow all rules imposed on users of the Park that are in place now or imposed in the future.
3. I understand that the Park is open from dawn until dusk and that I am prohibited from using the Park outside those hours.
4. I agree that I will not bring any dog to the Park unless the dog is well behaved and well socialized.
5. I agree that if my dog is hesitant to enter the Park, I will leave and, if I choose to do so, come back when the Park is less crowded.
6. I agree that I will not leave my dog unattended inside the Park.
7. I understand that my dog must respect the Park and all persons and other dogs at the Park at all times.
8. I agree to pay attention at all times to my dog while at the Park.
9. I understand that it is my obligation to know where my dog is and what my dog is doing at all times while he/she is at the Park.
10. I agree to be in voice control over my dog at all times.
11. I understand that if I am not able to control my dog by voice, I am not permitted to bring my dog to the Park.
12. I agree that if my dog is small or timid, I will not use the large fenced in area but will use only the special area to the left side of the large fenced-in area.
13. I will not bring any animals other than dogs to the Park.
14. I agree to leash my dog at all times until safely inside the Park and I agree to return my dog to a leash before exiting the Park.
15. I will make sure that the Park gates are closed after I enter and leave the Park.
16. I agree to enter and exit the gated area one at a time to promote safety.
17. I will not leave a leash on my dog while inside the Park.
18. However, I will always have with me a leash in case I need to place it on my dog in an emergency.
19. I understand that I must always remain in the fenced area while my dog is using the Park.
20. I agree not to bring any children under the age of 12 to the Park.
21. I agree that children ages 12 and above must be accompanied by an adult at the Park at all times.
22. I agree to supervise any children ages 12 and older at all times while they are at the Park.
23. I agree not to bring any aggressive or dangerous dog to the Park.
24. I agree not to bring any dog in heat to the Park.
25. I will not bring any dog to the Park with a history of biting or other aggressive behavior.
26. I will not bring any dog to the Park that has been deemed a “dangerous dog” under New York law.
27. I agree to immediately leash and remove my dog from the Park at the first sign of aggressive behavior.
28. I agree to prevent my dog from digging holes.
29. I understand that I will be personally responsible for any injuries or property damage that my dog causes while at the Park.
30. I agree to be courteous to all individuals and dogs at the Park.
31. I agree not to bring any sick dogs to the Park.
32. I understand that unlicensed dogs are prohibited from entering the Park.
33. I understand that dogs not vaccinated for rabies and DHLPP are prohibited from entering the Park.
34. I understand that all dogs at the Park must wear dog tags at all times.
35. I understand that spike collars shall not be warn by any dog at the Park.
36. I agree to promptly dispose of all waste from my dog in the designated on-site containers. I will seal the waste in a provided plastic bag or other bag before disposing it in the said containers.
37. I agree not to feed my dog at any time while at the Park.
38. I agree that, if I wish to give my dog water while at the Park, I will bring a dish specifically for my dog.
39. I agree that I will not bring any toys to the Park that my dog is not willing to share with other dogs.
40. I agree to dispose of all cigarette butts and other to***co products in the provided sand buckets.
41. By using the Park, I understand and agree that my dog and I are assuming all risks inherent in using a dog park, including, but not limited to, the risks of injury to myself and my dog and the risks of damage to my property.
42. I agree to indemnify and hold harmless the Village of Lakewood from and against all claims and losses related to or arising from my use or my dog’s use of the Park, including, but not limited to, all losses, claims, expenses, costs, judgments, awards, penalties, fines, and attorneys’ fees.
43. I understand that if I do not follow one or more of the above rules of the Park, the Village of Lakewood has the right to penalize me and my dog by banning me and/or my dog from the Park for a specific period of time or permanently, as determined by the Village of Lakewood Board of Trustees.
44. I have carefully read the above provisions of this contract and I have had the opportunity to discuss them with an attorney of my choosing.
45. I agree to be bound by the above provisions and I fully understand my obligations.
