There are several reasons why we did not go back to pre-Covid regarding entry and leaving our clinic.

The main reason is epidemiology: we are seeking to prevent and minimize infectious disease spread by restricting dog to dog contact. The goal is for your dog to never come into contact with another dog while inside the clinic.

Also, from an epidemiology standoint, this also minimizes risk of human to human transmission as well!

We also see some dangerous dogs here. Not bad dogs, but they can be dangerous. It would not be good if one of these dogs was making their way through the front vestibule at the same time that little Fluffy was coming in - it would not go well. This also includes people - some of our client dogs may become very aggressive to other people especially near their owners. Therefore, just walking in to pick up medications without calling is not advised (as demonstrated recently!!!!!!!!!!!)

Please be patient. This system works really well to ensure a mostly private experience for you and your dog while visiting True Care. The dogs very much appreciate this.

Dogs are notoriously bad at dissipating body heat. Watch for early signs of heatstroke, also known as hyperthermia, in your dog to avoid serious outcomes.

Canine infectious respiratory disease update!

Folks, this latest version of CIRD that has affected dogs in many states is likely a variant of an "old friend" - so-called "kennel cough". The major difference being that more dogs have developed fever and bronchopenumonia than you would normal expect, although these can occur with regular kennel cough syndrome.

The best advice if solid preventive care. If your dog must board or go to day care, go to the groomers or even dog parks, then ensure:
- all vaccinations are current and
- they are on solid year around parasite control

Dogs with pre-existing respiratory conditions or any condition which can tax their immune systems are at increased risk.

LEPTOSPIROSIS - rearing its ugly head again. And, of-course, there has been a lot of rain - it is sometimes called a "monsoon disease"

Leptospirosis is a world wide bacterial disease affecting humans and animals and can be transmitted back and forth. Generally, if your dog contracts leptospirosis, you are more at risk of contracting it from the same source rather than directly from the dog. The exception would be, an infected dog shedding the disease - if the urine contacts a sore or a mucus membrane, you can get the disease.

Vaccination is the best way to prevent the condition and, contrary to belief, the vaccine is well tolerated. All dogs in Western NY exposed to the following environments should be vaccinated:
- puddles, ponds, creeks, bird baths
- gutter run off
- very damp areas of soil
- anywhere a rodent can urinate

Dogs can be asymptomatic. When signs occur they can vary from simple lethargy (quiet) to fever, vomiting/diarrhea and lameness (muscle pain likely secondary to dehydration). The condition can be challenging to diagnose because these symptoms occur with a variety of diseases.

The disease can cause acute kidney failure - if this is not addressed soon enough, the condition likely will be fatal.

Allergic disease in dogs is extremely common and there are a plethora of allergic disorders for them to contend with. The triggers are numerous. Here is an example of one for which there is a suspicion that exists in dogs but has not been proven:

Urinary tract infections are much more common in female dogs than male dogs and not very common in cats at all

Signs of lower urinary tract disease include:
- increased frequency to go
- normal to smaller amounts passed
- leaking urine
- straining or sense of discomfort
- excessive licking at the external genitalia
- possible blood or blood clot
- decreased appetite and lethargy or hiding
- some dogs can get very sick

Some of these signs can be seen with upper urinary tract disease. They also can be seen with other conditions as well, such as cystitis (inflammation of the bladder), urinary stones, polypoid cystitis, emphysematous cystitis, bladder cancer or any one of several congenital conditions (of which signs would have been present since a very young age). Some medical conditions such as adrenal gland disease can predispose a dog to urinary infections, as can certain medications

If you are suspicious of urinary tract disease, here is what we recommend as a standard of care approach:
- submit a voided sample, especially a first am sample in a clean
container (this would not be cultured
- otherwise, retrieval of urine through cystocentesis would allow
a routine urine study, as well as a culture
- urine culture is the gold standard for the diagnosis of bacterial
urinary tract infection
- a fast scan of the bladder with ultrasound can help rule out many
of the above listed conditions

Cystocentesis is easier to perform in cats who can often sit upright for the procedure - at True Care, we never perform blind cystocentesis on any pet

It is less than ideal to culture a freely caught urine sample as there may be bacterial contamination from the vaginal cavity/male prepuce or even the container

Dispensing antibiotics without a culture result with sensitivity is not recommended, however, can be considered in cases in which urine is difficult to collect. Since antibiotics can potentially cause serious side effects, sedation should be considered. If a course of broad spectrum antibiotic is prescribed and there is:
- a response but then a return to clinical signs
- no response
then culture should be performed

Of note, bacterial urinary tact infections is a leading contributor to certain types of bladder stone in dogs

There are 4 orthopedic emergency situations:
- head trauma/injury
- closed fracture with progressive swelling of the leg
- open fracture with bleeding
- paralysis of a leg(s)

If your dog yelps (or not) and is limping but using or toe touching a leg, and there is none of the above occurring, then REST is BEST. Simply contact the office when we are open to discuss. If you call the emergency clinics they (of-course) will want your pet to be seen. And this can be justified to, if anything, dispense pain management. However, if your dog has ruptured (or partially ruptured a cruciate ligament (ACL) for example - there will be no surgery done immediately. Even fractures are medically stabilized until fracture repair can be pursued. The average cost of a fracture repair in this area is around $4000-$5000 not including after care.

Do NOT give human medications such as ibuprofen, You can get away with a dose of aspirin with a small meal (see our email contact below) - but not routinely. Assist for controlled bathroom walks and rest in between.

There are too many dogs being rushed to the emergency clinic, only to discover that nothing is broken and they could have been rested.

REMEMBER, the first step in any emergency is take a deep breath and remain calm - so that the correct, appropriate decisions can be made.
