Leo sustained a tibial fracture, which was repaired with a 3 mm interlocking nail and he clinically has done well. Typical in cats we do not pre-bend tibial nails in recurvatum, as we do in dogs. The powerful calf muscles can bend nails in the postop period. As you can see in the radiograph made at eight weeks postop, there is a mild bend of the nail at 3.5° procurvatum, which is not clinically significant. Pre-bending tibial nails in recurvatum, has a phenomenal protection against unwanted bending of the nail during the healing process. Perhaps we should prebend nails for feline tibial fractures, as this bone unusually can only accommodate a 3 mm nail.

Copper colored eyes, neurological signs and elevated bile acids are definitely grounds for a diagnosis of a Portosystemic shunt in this kitten. Surgery revealed a Portophrenic shunt and an ameroid constrictor was placed on the shunt. We wish Edward a speedy recovery.

Heston was bit in the head/ jaw by another dog when he was a puppy and his jaw bone subsequently fused to the zygomatic arch. When we saw him a few days ago he could not open his jaw at all. The zygomatic arch and the ramus of the mandible were removed using a pieziotome which is a bone cutting power tool that cuts the bone and spares vital structures like nerves and blood vessels. Heston is at home and happy to be eating his food again.

Kash gets a heart fix. This 4 pound cutie had a patent ductus arteriosus that was surgically ligated. I always do a cranial to caudal dissection around the PDA with right angle forceps. To date, 0% complication rate with intraoperative hemorrhage using this technique. With the traditional method, as described in textbooks, involving a caudal to cranial pass of right angle forceps around the PDA I have had a few patients who have had significant intraoperative hemorrhage. In such cases I have remedied this complication with a cranial to caudal pass of the instrument to save the patient. To date 0% fatality with this surgery; however, the procedure carries a risk of intra-operative death.

Tarsometatarsal luxation in dogs mandates performing a partial tarsal arthrodesis if the plantar ligaments are torn. Preoperative stress radiographs are necessary to determine the which aspects of the tarsus are unstable. Image 1: a medial stress radiograph demonstrates medial instability due to medial collateral ligament tear. Image 2: a plantar stress radiograph demonstrates plantar instability of the tarsometatarsal joint. Image 3: a dorsopalmar view of the tarsus; a small medial approach is used to access the tarsometatarsal joint, debride cartilage and re-establish medial instability with screws and encircling cerclage, thus realigning the tarsus. Image 4: plantar stability is achieved with a plate extending from the calcaneus bone to a metatarsal bone via a plantar approach. The distal aspect of the plate is placed over the 5th or 4th metatarsal bone. The 5th metatarsal lends itself to easy plate application with minimal plate contouring needed. Care must be exercised when placing screws in the region of the talus to minimize the risk of extension of screw tips into the talocalcaneal joint. Plantar plating is advantageous, as the plantar aspect of the tarsus is the tension side, thus allowing for smaller metal implants with minimal risk of implant failure. Postop casting of the tarsus is performed to protect the repair in the post period until fusion of the bone is seen on radiographs.

Beautiful Bailey is recovering well following a rim mandibulectomy to remove a tumor from her jaw. With clean surgical margins and a non spreading tumor, her prognosis should be great. She can't wait to get back to work outdoors.

Dr. Degner and staff,

I wanted to send you a quick update on Poppy. She was in this spring for surgery on her Superficial Digital Flexor tendon and is doing great now.

We followed up after our last appointment with you and went to MSU Rehab with Dr. Shull. At the end of September she was good to go and she just competed (softly) in her first disc event of the year. She was so happy to be back out in front of the crowd (or her subjects...LOL) and running after the discs.

I've attached a couple of photos of her at the event.

Thank you so much for the great care that she received from you and your staff. I cannot say enough good things about your facility and will always recommend you to anyone needing help.

I hope to never have to come back (ha ha), but I feel much better knowing you are there if needed.

Sincerely,
Diane
