THE PHYSICAL EXAM

“He doesn’t need a whole exam Doc, I just need you to look at his tooth.”

“I just want to drop off a urine sample, I don’t need to bring Missy in for an exam. She’s healthy, but her urine doesn’t look right.”

“Buddy is here for his leg Doc, why are you taking his temperature, he doesn’t like that.”

These are 3 very real examples of comments I get from well meaning owners EVERY. DAY.

Your dog with a tooth problem might need a dental and tooth extractions and therefore need to be put under anesthesia in order to properly do so- we want to know about the heart murmur that he has which might affect his safety under anesthesia or change our anesthetic plan & protocol.

Your dog with dark urine might be jaundiced and having severe liver problems - but we can’t see that if you don’t bring your dog into the building and let us provide a thorough exam.

Your dog with a limp who you thought just stepped on something might have a 104F fever and actually be limping due to a Lyme disease infection.

But I won’t know these things unless I do a complete exam. EVERY. TIME. I need to listen to those heart and lungs, examine the gums, take a temperature, palpate that abdomen, put my hands on the lump you noticed (not just look at a picture you emailed us).

As a small animal veterinarian, the physical exam is THE. MOST. IMPORTANT. diagnostic tool I have at my disposal. Not only is it the most useful, but it is the most cost effective. Our pets cannot use words so their body must tell us what the problem is.

Now, can I diagnose every condition from a physical exam? Absolutely NOT. But I can provide a solid differential in 90% of cases. What that exam also buys you is a diagnostic plan, possible prognosis, an estimate for further tests, and then a treatment plan. I will offer your pet the gold standard of care and if that is not within financial or physical reach, we will tailor that plan to your pet and your needs.

Sometimes your pet really is perfectly healthy and we don’t need to do anything else. Yay!! that’s great- I am truly happy in those cases. But PLEASE don’t disregard the value of the physical exam.

Cracked Window Myth

K-9 Rico is sad when he hears our Community Service Officers or Patrol Officers responding to a call with a dog in a hot parked vehicle. According to the American Veterinary Medical Association, cracking the window isn't enough to keep the temperature of your car at a safe level. If your dog is distressed and at risk of severe injury or death, that cracked window will be shattered if that's what we need to do to save your pet's life.

It's going to be in the 90s this week. Don't leave your pet in your vehicle, window cracked or not. Leaving your buddy at home is safer, and we all love coming home to a wagging tail! If you must take your pet with you, use grocery pick-up or do it during the evening hours when it's dark.

You also need to think about your buddy's paws when taking them for a walk when the sun is blazing. Asphalt temperatures can be more than 50 degrees warmer than the air temperature. Four Paws International has a simple test you can do to make sure the ground is safe for your four-legged friend. Touch the back of your hand to the ground for seven seconds. If you can't hold it there for seven seconds, it's too hot for your dog's paws.

"He's afraid of X, so he must have been abused by/with X."

In rescue, a lot of dogs come into care with unknown histories. Many of these dogs present with specific or generalized fear and anxiety.

It can be easy - so, SO easy - to attach a story to their behaviour. In fact, it's becoming increasingly common for people to adopt dogs BECAUSE of their story. The more tragic, the better.

The problem with this is that it tends to freeze dogs into their neuroses. Their owner becomes so attached to the story, so emotionally invested, that they themselves cannot move past it. When the owner gets stuck, so does the dog.

Then the story becomes an excuse.

He's aggressive to the postman because one must have abused him.

He's leash reactive to other dogs because he was a 'bait dog'.

He runs from us when we grab the fly swat because someone has hit him with it.

In most cases, these behaviours are caused from a lack of experience in the critical period of socialization. Their inexperience can present itself as fear, anxiety, stress and aggression; mechanisms they develop over time to cope.

But what exactly caused their rescued dogs behaviour is irrelevant. It's a fools errand to try to figure it out; an addictive oxytocin-fueled quest to justify a lack of action.

He's afraid of men? Make men = good things. He runs from the fly swatter? Pick the damn thing up and throw the dog a handful of steak a few times. See what happens.

When we adopt a dog, we're making a commitment to BETTER their life. Holding on to their past is doing the opposite. Show them they're in a better place by overcoming their problems, not nurturing them.
