What is a wellness plan?

A wellness plan is a way that we can make the services recommended by the doctors at Jelsema Vet Clinic more affordable for pet owners. It includes all the services that we recommend for your pets annual wellness care, payable over 12 months.

For dogs, this includes the following:
• 1 wellness exam (office call)
• Bloodwork-
◦ CBC
◦ Chem 18 (evaluates organ function)
◦ SDMA (early kidney disease screen)
◦ Heartworm 4Dx Test (tests for Heartworm, Lyme Disease, Anaplasmosis, and Erlichiosis)
• F***l Dx (detects common intestinal parasites and Giardia)
• Vaccines-
◦ Rabies
◦ Distemper (DAPPV)
◦ Bordetella
◦ Leptospirosis
◦ Lyme
• Heartworm Prevention
◦ 12 Months of Owners Choice- Proheart, Interceptor Plus, or Heartgard
• Flea/Tick Prevention
◦ 12 Months of Owners Choice- Vectra 3D, Nexgard, or Seresto Collar (2)

For Cats, The plan includes:
• 1 wellness exam (office call)
• Bloodwork-
◦ CBC
◦ Chem 18 (evaluates organ function)
◦ SDMA (early kidney disease screen)
• F***l Dx (detects common intestinal parasites and Giardia)
• Vaccines-
◦ Rabies
◦ Distemper (FvRCCP)
◦ Feline Leukemia
• Flea/Tick Prevention
◦ 12 Months of Owners Choice- Revolution, Catego or Seresto Collar (2)

Plans also include a 10% discount on all surgeries, medications, supplements, and diagnostics during the plan year. The only exclusions are veterinary diets and online pharmacy purchases.

Plans offer savings of $180 to $375 over purchasing items individually. Savings depends on size of pet.
