Back to our pericardial case: We relieve this pressure with a pericardiocentesis to remove the fluid. This fluid is most commonly hemorrhage/blood however doesn't always have to be depending on the underlying cause.
A wide variety of symptoms that are not always specific however, can all point in the direction of pericardial effusion. This can include (not limited to) acute vomiting, collapse, difficulty breathing, weakness, lethargy and coughing. This dog had both pleural (free fluid in chest) and pericardial effusion. As you can see in this video - there is fluid within the pericardium which is the normal sac that is around the heart. This is most commonly due to a bleeding tumor on the heart however, can be a few other less common causes such as infection, other types of cancer, trauma, coagulopathy or rupture of the left atrium. This leads to pressure within the pericardium which compromises the hearts pumping ability called cardiac tamponade ... Stay tuned for treatment and outcome ...

Multimodal anesthesia for the win! This is a femoral/sciatic block for a TPLO (Like ACL surgery in people). When incorporated into the anesthesia plan, it is opioid & MAC sparing. This pup was so comfortable during and after surgery!!

The EKG for our sick sinus syndrome! Check out what you were seeing on the tFAST/foCUS echo!
Ever see a heart stop beating in a live pet?!! If you watch this ultrasound closely, you can see the heart skip beats for long periods of time. This dog had something called “sick sinus syndrome” which is a cardiac abnormality that can cause the pacemaker in the heart (the sinus node) to occasionally fail to induce a contraction. This can lead to weakness or collapse and can sometimes require a pacemaker to be implanted, just like in people! Luckily, this pet was able to compensate for this condition and did great with simple medical management.

Sometimes our dogs and cats come up with a limp out of nowhere! In many cases the answer is as easy as a strain or sprain, but sometimes we have to look further. In this case, the limp wasn't getting better and no bone/orthopedic abnormalities were identified, so we had to take samples from the joints. Evaluation of joint fluid allows us to assess for signs of infection, inflammation or immune-mediated joint disease. Dogs can get these conditions just like people! This little pupl is feeling much better now that we found out the secret!

Thankfully this stick stayed very superficial - just within the skin and muscle - rather than penetrating into the chest cavity! This is why it is so important to remember things happen quickly while outside on an adventure as well as it may look superficial but may be the tip of the iceberg! Do not remove on your own - the object or stick may be keeping pressure on major blood vessels or into the chest or abdomen. All of these injuries can become rapidly life threatening within a second!

Removing a quill from the lung lobe that was causing air to leak out into the chest which is called a pneumothorax. This portion of the lung lobe also needed to be removed due to the damage. This air leak can quickly become a life threatening emergency.

An important piece of assessment on triage - a quick tFAST/aFAST! This can give you so much information very quickly and non-invasive. In this case, pleural effusion and an intrathoracic mass. #vetblue Rarebreed Veterinary Partners Maine Veterinary Medical Center
