Let's talk about this hind hoof, the purple shows the forces that are contracting the frog. Green shows this very deep central sulcus thrush, blue shows the curling crushing heels. Yellow shows the useless no account no job frog.
The problem I find is not my ability to improve the hoof which I can, but results may not be fast enough or the horse may not be able to perform like the human expects. The human might get contrary advice from well meaning friends, trainers or other boarders.
Improving a hoof takes a good frequent trim, movement and good nutrition. You may be able to shortchange in one or both movement and nutrition which may limit or halt improvement but the frequent correct trims are absolutely critical. This hoof like the other 3 is like this from incorrect stimulation of the structure. The hoof needs to function with all aspects in alignment. It's a balance of pushing the hoof just far enough but not too far. It really is art, intuition and knowledge.

This is an older pony that had large fat pads and he had very funky feet. He's also got Cushing's and he's being managed well with his diet now. What's interesting is it's been difficult to get huge improvement as he has severe deformed twists in both fronts with impacted melded welded sole that has old serum embedded in it. I finally got the jammed up Coronet and cartilage to relax and straighten out. The blue line shows this. The purple heel is obviously improved but white dorsal wall is still going to have to grow out and you can't see the twist from the side view which we are still working on. He's moving very nicely during this transition.

This matches what I've observed without being consciously aware of this. Connecting some more dots.

Laminitis, one of the dreaded words we don't want to hear associated with an equine in our care. This goes for the hoof care provider also. The first question we are asked is how?
Diet is the main culprit with the easy keeper types at a higher risk of developing it. I've watched healthy clients slowly develop little red flags, I will alert the human and it will be up to the human to discuss with their vet on the next steps.
Fiber and forage are the primary very easy fixes, cutting starchy and sugary grains are absolutely critical. This doesn't mean starving the horse it means to mimic what we can in nature, slow feeder hay nets 24/7 to give them access to forage to buffer their guts and provide slow steady energy.
Movement has also been shown to keep the metabolic types with healthier levels.
This pony was a rescue and we don't know her past few years but I can tell her feeding was incorrect and exacerbated by an incorrect approach. The heels were allowed to grow which you can see in the diverging rings in the heel, the toe starts to curl upwards as the physical forces lever internal forces. One theory is the hoof knows it's supposed to land heel first as that's the shock absorbing function so it grows more heel in an attempt to help. If the pony or horse is landing toe first then the heel does not get any stimulation and grows. The toe does not grow at the same rate, this is again evident in the rings in the hoof.
Step 1 Consult with your vet.
Step 2 Evaluation of diet with necessary changes.
Step 3 Frequency of trims to make effective changes
Step 4 Working with a body worker to release tension patterns in the body.
Step 5 Try to get slow steady movement daily. Using enrichment techniques like hay nets with different locations and changing terrain. Even if it's putting up temporary runs with ground poles to navigate around or over.
Step 6 Document and keep a diary to track improvement as you can become discouraged. Track pain level in tension around face and body. How much are they moving and the grading of movement. Pictures your hcp will take pre and post trim will help you.

Laminitis can affect hinds as well. Typically we see the fronts affected and the hinds to a lesser degree. You can see in these hooves the very jacked up heel, the dorsal toe wall is not touching the ground. The cannon bone shows a very camped under posture.
