We are trying to save the pets life and need the information - +/- concerned about persons decisions in life (we care about all life but in this situation the pet is our concern).

Doctor Hubbs’ Insight:
Prescriptions are the bane of my veterinary career. As a veterinarian, we went into the profession to “practice” Veterinary medicine and now we are forced to be Pharmacist in addition to doing internal medicine, dentistry, surgery, reproduction, lameness, etc. You get the point. Prescriptions take up almost 1-2 hours of a working day! Why do you ask? Let me explain it to you…
Controlled substances and monitored bloodwork: Let’s use Phenobarbital as our first example. The pet is on medication for seizures, and we have now gotten the pet balanced. That means for NIAH, that the pet has a Phenobarbital level done 9-12 months (if no major changes) and the owner receives a prescription for the Phenobarbital to receive enough medication to last 31-34 days. With 1-4 refills (by law what is allowed) then we will write a second script for the pet and only go to 5-10 days prior to their yearly bloodwork to test Phenobarbital level. By law, I cannot continue to refill medication on a pet that: 1: Doesn’t have an up-to-date examination 2: has no more refills on their medication 3: Doesn’t have a current blood test value in their record 4: Is under another Veterinarian care. Phenobarbital is a controlled substance that requires monitoring and supervision on prescriptions.
So, let’s say we have a pet needing more arthritis medication. The pet may be on either Tramadol or Gabapentin in addition to other medication (s). First, BOTH drugs are now on the controlled substance monitoring system in the State of North Carolina. Why do you ask? Humans like to take, share, and use their pet’s medication. Really? YES. We have clients that come in all the time that have given a controlled substance to another client’s pet, or their own medications. The controlled monitored substances cannot be controlled and monitored if we do not know where they came from. The second reason these two are a royal pain in the butt to write prescriptions for is that I must calculate based on the administration we have established, not our owns (for use on their pets). Give 1 tablet by mouth 2x daily (not 3 or 4 x one day and 3 the next). So, when I write the prescription it is based how the pet is “SUPPOSED” to take the medication, I must calculate the number of days the pet will take the medication, when the medication will run out, and allow enough time for the pharmacy, shipping pharmacy, and prescription to arrive.
IT IS NOT MY FAULT IF YOU RUN OUT, DO NOT HAVE BLOODWORK COMPLETED, OR YOU DID NOT ORDER YOUR PETS MEDICATION IN TIME TO AVOID ABSENT DOSES.
I must work approximately thirty minutes to make up for the time I spend writing one to two prescriptions. For an owner to drive in, pick up the written prescription, to drive to the pharmacy to pick up the prescription (Some we do not carry for the above mentioned gets about ten times more complicated), when they are saving maybe $8.00 per prescription. This is the main reason why we stopped doing third party on-line pharmacy work and we charge for the prescriptions here at NIAH. It takes valuable time and money away from medicine to do all the prescribing.
So, when you must wait for veterinarian care you can thank pharmacy obligations. I have established guidelines that assure I do the prescriptions one time during the day (yes you may have to wait – but if your pet was in the exam room wouldn’t you rather take precedence?). I pay very close attention to the pet that is in front of me and do not tend to other things. I focus on right now and present; we hope you can appreciate what is involved in cost, time and prescription for pet.
