🛑 Let’s talk about linear foreign bodies! 🧶🐾

Keeping up with the latest literature 📄 is a challenge when balancing life 🐈🐕👶 and busy days on clinics. 👩‍⚕️👨‍⚕️🐾 We're all in this 🛥 together. Keeping it practical and 🩺 clinically applicable to veterinarians in all aspects of small animal 🐱🐕‍🦺 practice is our mission!

Ribbon or 🧶 string, 🪡 thread or dental floss in 🐱 is the classic one that we all think of. This is why all cats should have an oral exam being sure to evaluate under the tongue to rule out an anchored linear foreign body. However, 🐕‍🦺 commonly have linear foreign bodies as well. Rope toys 🪢 are classic culprits. Hair, socks, other fabric 🩲, hair and grass has even been implicated.

➡️ In the acutely vomiting patient, abdominal radiographs are often the first imaging technique utilized. Although ultrasound machines are becoming ubiquitous in most practices for POCUS, cystocentesis and other brief ultrasounds, diagnostic abdominal ultrasound is often not readily available 🕑 in most general practices, at urgent care centers or after hours in many ER & referral hospitals.

🩻 Often, we rely on serial ⏳ radiographs - a recheck set of films at 8-24 hours - to assess for improvement in the gas pattern or progression, in order to diagnose a surgical abdomen.

✅ Abdominal ultrasound, with an experienced sonographer, has the highest diagnostic accuracy for confirmation of mechanical obstruction, linear foreign body as well as having the benefit of evaluation for effusion and evidence of regional peritonitis that increase concern for perforation or necrosis and resultant septic abdomen.
✅ When available, abdominal ultrasound should be completed in clinically unstable patients, those with decreased serosal detail, patients with acute abdomen, vomiting patients with fever, and those with equivocal radiographs.

🩻 For those veterinarians without access to abdominal ultrasound, what studies are there that look at the utility of ⏳serial radiographs as a diagnostic tool for determining a surgical abdomen vs. a non obstructive or partially obstructive - and medically manageable - foreign body?

📄 Summary of Recent Literature on Radiographic Monitoring in Abdominal Foreign Body Cases:
✍ A prospective cohort study of 57 patients (40 dogs, 17 cats) was conducted to evaluate whether follow-up radiographs increase diagnostic accuracy for mechanical obstruction. 19 of the cases were mechanically obstructed with non-radiopaque foreign material. Initial radiographs were equivocal for obstruction and follow up radiographs were obtained between 7h and 28h later. All patients had abdominal ultrasound completed within 3h of the follow up radiographs. There was no significant change in accuracy (P = .058-.87) for the diagnosis of mechanical obstruction when the initial radiographs and initial & follow up radiographs were reviewed by the radiologists or criticalist. Given the lack of significant increase in diagnostic accuracy using follow-up radiographs in cases of occult gastrointestinal mechanical obstruction, abdominal ultrasound could be considered if radiographs are equivocal.

📄 Summary of Recent Literature on Linear vs. Discrete Foreign Bodies:
✍In 2023, a retrospective study was published looking at treatment of 56 cats with linear or discrete gastrointestinal foreign bodies at one university site. 18 of the cats had linear FB and 38 cats had discrete (non linear) foreign bodies. Cats with linear foreign body had a higher ASA Status (https://www.avtaa-vts.org/asa-ratings.pml), a longer surgery length, higher cost 💵 of care, and were more likely to develop surgical site infections. All cats survived treatment.

✍In 2014 a large retrospective by Hobday et al. looking at 499 dogs with linear or nonlinear foreign bodies was published in JSAP. Dogs with a linear foreign body were more likely to have a history of vomiting, anorexia, lethargy and pain on abdominal palpation. They were also more likely to have the foreign body anchored in the stomach and continuing into the small intestine, experience intestinal necrosis, perforation and peritonitis, and require intestinal resection and anastomosis. The duration of hospitalization was longer for dogs with linear foreign body (3 days vs. 2 days), and the cost of treatment was higher. Survival in this study was identical between the 2 groups (96%).

✍A 2009 JSAP retrospective of 208 cases of gastrointestinal foreign bodies in dogs and cats identified that survival rates were lower for both dogs (80%) and cats (63%) with linear foreign bodies than those with discreet gastrointestinal foreign bodies (dogs 93%, cats 100%). The duration of clinical signs, presence of a linear foreign body and need for multiple procedures was associated with a higher risk of mortality. The location of the foreign body was not associated with mortality.

⏰ Clinically Relevant Literature Update

What single blood value should never be evaluated alone? ✋✋

Packed Cell Volume (PCV)

The total solids (TS) tells so much (more) about the patient, particularly in acute cases - trauma, hypovolemia and hemoabdomen. PCV should ALWAYS be evaluated with the TS.

There is a new 2023 study which confirms the clinical importance of evaluation of the TS with every PCV. ✍️

📄 In this 2023 study in JVECC by Buseman et all (https://doi.org/10.1111/vec.13343) presenting PCV/TS was evaluated along with APPLEfast scores and standard intake bloodwork (lactate, venous blood gas, CBC, chemistry) in 90 dogs with hemoabdomen.
- 64 dogs = non traumatic hemoabdomen
- 26 dogs = traumatic hemoabdomen

🛑 For ** each 1 g/dL unit decrease in TS ** on presentation, dogs were approximately ** 2 times more likely ** to receive a red blood cell transfusion during hospitalization.

🛑 Low admission total plasma protein (TS), independent of low PCV, was associated with red blood cell transfusions regardless of underlying cause.

Photo credit: Dr. Welch's dog, Boston, who suffered from hemoabdomen secondary to a ruptured right adrenal tumor and lived 13 amazing months after transfusions and emergency adrenalectomy. He refused a piece of chicken one night (what lab refuses food??), was evaluated in hospital immediately - his PCV was 43% on presentation but with a TS of 4.3 g/dL and he had a large volume hemoabdomen.
