Wounds that pe*****te into the digital tendon sheath can be life threatening.

Not only did this little lady cut into her digital tendon sheath, she cut into the superficial digital flexor tendon, which on its own can be career ending.

The odds were not in her favour. The referral surgeon said if she didn’t go to surgery, she would need to be put to sleep, and even with surgery, her chances of making a recovery for an athletic career were poor.

The trainer decided to treat the mare conservatively with box rest, bandaging and some antibiotics and anti inflammatory’s, but was completely aware that she may not make a racehorse, let alone survive this type of injury.

It’s just amazing what a bit of time (and some Manuka honey!!) can do. The pic on the left is the injury at one of its earlier bandage changes, and the pic on the right is from today, six months post injury and back at the trials ready to race again.

Sometimes miracles do happen, and this is one very lucky little lady that the trainer took a chance on her ☺️

Normally when we reach for the ultrasound in equine practice, we are scanning a tendon or ligament. But ultrasounds are great for any soft tissue imaging, whether that be assessing the lungs, looking at the abdominal organs, scanning the reproductive tract or in this case, imaging lesions in the eye.

The owner had noticed a mass in the eye of the horse (red arrow). The main differential diagnoses were a tumour (melanoma) or a corpora nigra cyst (a cyst from the brown part of the iris).

On ultrasound a tumour would appear as a solid mass (a solid white circle), whereas a cyst would appear like a fluid filled ball (white outline with a black inside).

The images showed this to be a cyst (blue arrows).

Corpora nigra cysts are not painful and slow growing, but the can get large enough to interfere with vision, causing the horse to be spooky.

Consultation with a veterinary ophthalmologist gave the option of having laser surgery to remove the cyst.

Edit: Donna Aspin was the first correct guess. There is swelling of the sinuses (the maxillary sinuses) under the left eye, the right of the picture. The red arrows point out the swelling.

The filly had had a left sided nasal discharge for several weeks. Endoscopy revealed that the discharge was coming from the sinuses and not the respiratory tract. It was suspected she may have a tooth root abscess, however radiographs revealed a mass in the sinus. Biopsy/sampling is being performed this week.

💡💡💡What you can’t see you can’t diagnose, and what you can’t diagnose you can’t treat💡💡💡

This pic highlights the importance of a sedated dental exam with a light and mirror.

This little guy had some minor swelling of the left cheek. Everything else seemed to be ok with him, but a couple of days before his dental appointment, he had started to go off his feed a bit, so his owners thought he might have stomach ulcers.

He has an ulcer for sure, but not in his stomach. The arrow is pointing out the large painful ulcer this guy had developed at the back of his mouth due to a sharp point on the back of his bottom left molar.

We reduced the sharp point on the molar and floated the rest of his teeth.

This just goes to show how important it is to have proper examination of the teeth done by a dental vet.
