Ultrasound examinations have traditionally been carried out by Veterinary Surgeons, but in recent years, there has been an increase in the number of Veterinary Nurses who have been keen to get involved, and VVS are delighted to be working alongside some incredible RVNโ€™s who are really enjoying scanning with us. ๐Ÿ‘

Did you see our Clinical Conundrum?

A 2-year-old German Shepherd presents to you with a chronic history of poor body condition score and weekly vomiting. Based on the history and the unremarkable bloodwork performed so far, you consider diet-responsive enteropathy to be a likely hypothesis. What do you do?

a) Test for serum food-specific IgE
b) Test for serum food-specific IgG
c) Gastro-duodenoscopy and gastrointestinal biopsies
d) Food elimination trial

Now we are revealing the answers...

The answer is.... d)

Unfortunately, testing for food-specific serum IgE and IgG is unreliable in dogs and cats and should not be used to diagnose a diet-responsive enteropathy nor determine what ingredients should be avoided when feeding a patient.

Biopsies can show inflammation, but cannot help differentiate between diet-responsive, steroid-responsive or antibiotic-responsive enteropathies.

Only a diet elimination trial is a reliable tool to confirm or exclude the hypothesis of a diet-responsive enteropathy.

Laura, tell us a little bit about your RVN journey prior to joining VVS?

I qualified from Hartpury University with a BSc(Hons) degree in 2019, and I worked at a small animal busy first opinion practice. I am an education lover, so not long after I developed my passion for surgical nursing and schedule 3 procedures and gained my NCert in surgery. Following this, I then worked at a university referral hospital as deputy head theatre nurse, and not long after I started an MSc in advanced veterinary nursing with analgesia and surgery specialties โ€“ due to graduate next year!

What do you love most about being an RVN?

I love the knowledge it has given me, and the ability to constantly re-evaluate, reflect and tailor the care and support our patients receive. I find it fascinating how quickly science โ€“ the building block of what we do โ€“ evolves and is constantly improving. Itโ€™s exciting how much potential we still have to unlock to keep providing the best care we can for our animals.

Suzanne, tell us a little bit about your RVN journey prior to joining VVS?

I qualified in 2007, and Iโ€™ve worked at Glasgow University Small Animal Hospital, an exotic practice (working with a White Tailed Sea Eagle was incredible!) and was then head RVN at a small animal practice for 9 years. I led an amazing team of nurses and enjoyed running nurse clinics and training SVNโ€™s.

What do you love most about being a RVN?

The feeling when you see a patient go home after being nursed through a critical illness is pretty hard to beat. Nurses really are involved in all stages of a petโ€™s journey, from that first puppy check, right through to euthanasia, and itโ€™s a real honour to be a part of it.

This years theme for VN Awareness Month is Progression. How has working at VVS allowed you to progress?

Iโ€™ve been qualified for 17 years, and Iโ€™ve seen a lot of changes in the Veterinary profession in that time, especially in Veterinary Nursing, and when I first came across VVS, I was instantly impressed! To be able to bring the Specialist into all Veterinary practices, no matter their location, is incredible. To be able to see the direct impact of what we are doing and how it benefits the patient, their owner, and the Veterinary team, is just amazing. Iโ€™m now in a position where I am working alongside amazing Vets and Nurses in practice as well as the fantastic team of Veterinary Specialists and using technology to provide the very best care for our patients. Working as a referral coordinator has allowed me to use all my preexisting Veterinary Nursing skills, and to learn new ones too. The beauty of being a RVN, is that we are always learning and bettering ourselves, and VVS has provided me this opportunity.

What do you love most about being a RVN?

I have always loved the variety that the job brings, no day the same, and you can never quite predict how the day will go!

Working for VVS has allowed me to use my experience but apply it differently. It has also given me the opportunity to explore other interests and expand my knowledge and skills. I joined VVS two years ago, and prior to this worked in a busy first opinion practice in sunny Cornwall. For a big part of my time there, I was a sole charge night nurse. I did really love my nocturnal years (crazy I know)! ๐Ÿฆ‰

What do you love most about being a RVN?

I love the friendships I have built through my career, nothing brings you closer to your colleagues than literal blood, sweat and tears ๐Ÿ’•

This years theme for VN Awareness Month is Progression. How has working at VVS allowed you to progress?

I have completely diversified my career in a direction I would not have known was possible before joining VVS. If you had told me I would now be working from home, helping vets all across the world access specialist level care I would never have believed you. I have built a whole different skillset to that which I had in clinical practice and I have loved my journey so far ๐ŸŒŸ

Oesophagostomy tubes can provide enteral nutrition to patients who are unable or unwilling to eat, therefore improving their overall outcome.

Feeding tubes are safe to place and associated with a low rate of major complications.

Glucocorticoids and chemotherapy agents are associated with an increased risk of stoma site infection in cats with an O-tube (https://pubmed.ncbi.nlm.nih.gov/31001901/).

Maintaining the tube for a prolonged period and discharging the patient with an O-tube does not increase the risk of infection, making O-tubes an ideal assisted enteral nutrition tool for patients with chronic disease or diseases associated with prolonged recovery.

b + c) Transdermal mirtazapine and oral mirtazapine.

A study by Quimby & Lunn (2013) showed that oral mirtazapine improved appetite, reduced vomiting and increased weight gain in cats with naturally occurring CKD. The evidence for transdermal mirtazapine is also good, with a study showing mirtazapine-treated cats gained 3.9 ยฑ5.4% body weight over 14 days which was significantly greater than placebo (0.4 ยฑ3.3% weight gain). In comparison, whilst maropitant significantly reduced the frequency of vomiting in one study (Quimby et al. 2015), it had no effect on appetite score or body weight. Finally, omeprazole did not affect body weight or other subjective assessments of food consumption in a small study by McLeland et al. (2014).
